- Purpose and Objectives: The intent of this Sources Sought Notice is to identify potential businesses, especially any Small Businesses, Service Disabled Veteran Owned Small Businesses (SDVOSB) or Veteran Owned Small Businesses (VOSB) offerors capable of providing A Dental Laser system and supplies as listed in the below Statement of Work utilizing NAICS Code 339114 (Dental Equipment and Supplies Mfg.). Responses to this Sources Sought Notice should demonstrate the firm's ability, capability and responsibility to provide the principal components of supplies listed below. Responses should include the following information: Business name, address, Point of Contact, Service-Disabled Veteran Owned Small Business info and if they are a manufacturer of the items. All information is to be submitted via e-mail at ronald.kline@va.gov. Information provided will not be returned. All responses shall be in the English Language. Responses are due by 3:00 pm (EST) on Tuesday, March 2, 2021. No submissions will be accepted after this date and time. Questions can be submitted electronically to ronald.kline@va.gov. This is a Sources Sought Notice and submissions will be used for informational and planning purposes only. This notice does not constitute a formal Request for Quote (RFQ), nor is the government obligated to issue an RFQ. In addition, the Government does not intend to pay for any information provided under this notice. The Government is not obligated to notify respondents of the results of this survey either. Page 4 of 4 STATEMENT OF NEED Introduction Veteran Affairs Healthcare System of Lebanon has a requirement to purchase one (1) Epic X TDS Dental Laser, one (1) Epic X Standard Surgical handpiece and accessories. See salient characteristics below. This requirement is in support of the Lebanon VAMC thirteen (13) dental clinics. This is a brand name or equal to requirement. In addition, the vendor will provide service manuals that includes theory of operation, maintenance requirements, repair information, schematics, part lists and troubleshooting information in electronic format for use by the VA Biomedical Engineering Department. Date of manufacture must not be greater than 2 years before the purchase order date. The equipment must not be used or refurbished. Any batteries provided with the unit will not be manufactured more than 12 months before the purchase order issue date. Lebanon, PA 17042 Delivery Schedule Items shall be delivered on or before 11/30/2021 2.2.1 The awardees actual delivery dates will be confirmed by VA upon award. 2.2.2 Post-award, the vendor will coordinate delivery identified in this document with Bio-Med, and/or the COR. Once the dates have been confirmed, modifications to the schedule are subject to written approval by the CO, COR and the vendor. 2.2.3 The delivery of items identified in this document shall take place during normal loading dock business hours which are defined as: 8:15 AM to 3:00 PM Central Standard Time, Monday through Friday, and excluding Federal Holidays. BioMedical Engineering will need to perform a safety check and enter the equipment into the system for annual Preventive Maintenance (PM) checks. 2.2.4 Labeling of delivered items shall include the VA facilities contract number and VA purchase order number for identification and reference upon receipt of product.
