- Department of Veterans Affairs Network Contracting Office 16 - HOUSTON In Support of the Overton Brooks VA Medical Center (OBVAMC) REQUEST FOR INFORMATION 36C25621Q0485 STATEMENT OF WORK Overton Brooks VA Medical Center (OBVAMC) Hologic T-5000 Cytology Processor BACKGROUND The Overton Brooks VA Medical Center located at 510 E. Stoner Ave; Shreveport, LA 71101 is requesting Assy Benchtop T5000 for the Pathology and Laboratory Service. The purpose of this purchase is to replace our current GYN and Non-GYN liquid based monolayer slide processing instrument. SCOPE To provide an instrument that can process at minimum 20 Non-GYN or 20 GYN liquid based monolayer slide preparations for screening and diagnostic purposes. The Instrument shall not exceed 36 in width and 30 in depth. The instrument shall not exceed 50 in maximum operational height (loading, unloading and processing patient samples). All samples and consumables must be front loaded due to processing space requirements. Our current Cytotechnologist and Pathologist are Hologic Certified for this instrument in regard to reading PAP Smears. The Vendor shall provide for purchase the requested liquid based monolayer slide preparation instrument used for GYN and Non-GYN sample processing. The Vendor shall provide and additional year of service and maintenance beyond the initial year. The Vendor shall be responsible for installation, set-up and calibration of the instrument. The Vendor shall provide local on-site training for all staff for proper use and care of the instrument, especially preventive maintenance. The base of the instrument shall not exceed 36 inches in width, and 30 inches in depth due to space limitations. The instrument shall have the capability to hold no less than 20 GYN or 20 Non-GYN samples at any given time. The instrument shall use a barcode system to ensure that the slide and patient sample vial match. The instrument shall be automated load and go The instrument shall be compatible with our current supply of ThinPrep supplies (filters, slides and specimen vials). The instrument shall be able to process both GYN and Non-GYN patient samples. END OF SATEMENT OF WORK REQUESTED RESPONSES: The intent of this Request for Information is to establish sources to define the procurement strategy (e.g set-aside, sole source, unrestricted) for a solicitation that VA intends to post soon. Please respond to this RFI if you can provide supplies that may be determined EQUIVALENT to the products listed in the table in the background section above. Please provide details on the proposed EQUIVALENT products such as Manufacturer Name, Part Number, and Description. If you have an existing GSA or VA, Federal Supply Schedule contract, please include the contract details in your response. DISTRIBUTORS: You must provide proof that you are an authorized distributor. VA does not accept grey market items. Please note that VA is particularly interested in determining the availability of Small Business Manufacturers. If your company is a small business manufacturer of potentially equivalent items, please respond to this RFI. Vendors are requested to submit estimated market research pricing with their responses. The estimated pricing will be considered when determining the procurement strategy for the future solicitation. (e.g. if CO determines that capable small businesses cannot provide fair and reasonable pricing, then the solicitation will not be set-aside). Please note that if no responses to this notice are received, from either authorized distributors of the cited brand name nor from manufacturers marketing a potentially equivalent brand, then this action will be sole sourced to the manufacturer cited in the Background section above.
