Description
(i) This is a Combined Synopsis/Solicitation for the purchase of Astera Audiometers, as prepared in accordance with the format in Federal Acquisition Regulations (FAR) Subpart 8.4, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. A firm-fixed price purchase order is anticipated. (ii) The solicitation number is 36C24421Q0353 and is issued as a request for quotation (RFQ). This solicitation is for a Brand Name Only Requirement. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2021-03 dated 1/14/2021. (iv) This solicitation is set aside 100% for Service Disabled Veteran Owned Small Businesses (SDVOSB) Set-Aside and the associated NAICS code 333912 has a small business size standard of 500 employees. (v) Statement of Work and Salient Characteristics Lebanon VAMC 1700 S. Lincoln Avenue Lebanon, PA 17042 Background: The Lebanon VA Medical Center has a requirement for the brand name purchase of four (4) Brand Madsen Astera 2 Audiometers with two-channel clinical high frequency audiometers and installation. The audiometers will be used to upgrade the current Otometric audiometers. Title of Project: GN Otometrics Astera 2 Audiometers Lifecycle replacement. Scope of Work: The vendor shall provide all resources necessary to accomplish the deliverables described in this statement of need (SON), except as may otherwise be specified. The contractor shall provide and install (4) GN Otometrics Astera 2 Channel Clinical High Frequency Audiometers as outlined. Background: The Lebanon VAMC Audiology department currently has a need to upgrade their Otometrics audiometers with (4) GN Otometrics Astera 2 Channel Clinical High Frequency audiometers which operate off the existing Otometrics OTOsuite software and hardware. We currently use the Aurical, Aurical OTO Cam, and the Madsen Capella OAE which are three other components sharing the OTOsuite software.   Performance Period: Delivery shall occur within 30 days of contract award. Type of Contract: Firm Fixed Price Delivery Order General Requirement: The Madsen Astera² offers a large range of dedicated tests such as the TEN test and SAL test. The TEN test lets you easily mask the frequencies next to the test frequency in the test ear. That way you can avoid off-frequency-listening and get a proper estimate of the hearing threshold. You may also get indications of whether it is worthwhile to provide amplification in that frequency region. The SAL test is based on an indirect method of estimating hearing thresholds for bone conduction. Hearing thresholds for air conduction are determined first without and then with a simultaneous masking noise, presented by a bone vibrator placed in the center of the forehead. The better the sensorineural acuity the larger the masking effect of the boneconducted noise in that ear. Madsen Astera2 also includes the SISI, ABLB, DLI, Bekesy and loudness scaling tests. The Madsen Astera 2 also shares the same existing software (OTOsuite) that the audiologist is currently utilizing for additional applications with patients. The main hardware is a direct connection to the computer via USB. The existing connected PC controls the entire system. There is no embedded operating system or personably identifiable information. Salient Characteristics: High Frequency Phones, TDH earphones, Insert Phones, Bone, Monitor Mic, TB Mic, Pat Response switch & Preloaded speech test. Compatible with Otosuite. Sole Source Justification: The existing software (OTOsuite) is not compatible with other audiometers on the market, and the current software is not in need of replacement, therefore requiring the upgrade to the Madsen Astera 2 audiometers for seamless installation, connectivity and functionality with the current software. Installation: The vendor shall provide installation within 30 days of contract award or on the day of delivery. Delivery: The Contractor shall deliver all equipment required under this SOW within thirty (30) calendar days or less from the date of award, unless otherwise directed by the Contracting Officer (CO). Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM. All software licensing, warranty, and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions. The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right to enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.
