B.2 PERFORMANCE WORK STATEMENT (PWS) Home Oxygen Services Part 1 General Information General: A single award will be made to the vendor whose offer meets all of the Government requirements and represents the best value to the Government considering price and technical factors. The contract will result in a sole source Bridge contract for one year. Awaiting the VISN 4 West contract to be awarded. Description of Services/Introduction: The Contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to provide Home Oxygen as defined in this Performance Work Statement; except for those items specified as government furnished property. The services associated with this contract include: All logistical/administrative functions involved with the day to day operation of providing the service to the Veteran and their families, including furnishing warehouse space, surplus supplies, and in home respiratory therapy visits. This includes providing a Quality Control Plan (QC) plan that is overseen by the Prosthetics Quality Assurance Evaluators (QAE), coordinating and managing patient care, deliveries consisting of pickup and delivery of supplies, briefing patient on proper use of equipment, documenting use of supplies and inspection of home in relation to home oxygen in accordance with accredited institution concerning patient use/safety of home oxygen standards. The need includes providing in home supplies such as the following: Continuous Positive Airway Pressure (CPAP)/ Bi-level Positive Airway Pressure (BIPAP) machines and replacement parts; Concentrator (Rental) with two broad categories' of flow rates and replacement parts; various cylinder sizes (rental) to provide oxygen [Ex. A (smallest)-E (largest)] and replacement parts; Liquid Oxygen; Oxysafe Firesafe Cannula; Procurement/Distribution of two types of oxygen and backup/spare parts; Nasal Cannula/Masks and replacement parts; Portable oxygen systems (used when a Veteran travels); Delivery of VAPHS owned equipment CPAP/BiPAP/Portable Oxygen Concentrator (POC)/ Oxysafe Firesafe Cannulas; contractor is responsible for delivery and associated services related to delivery/proper use of equipment. Contractor is not required to procure any brand name items such as Oxysafe Firesafe Cannulas. Background: The acquisition of Home Oxygen services is being initiated to provide Home Oxygen services to Veteran beneficiaries serviced by VAPHS . VAPHS does not have the capacity to provide these services so a contract has been sought for the provision of home oxygen and all associated products and services. Objectives: Provide Home Oxygen services for VA outpatients. Services include delivery, set-up, education, pick-up, cleaning , preventative maintenance, and storage of equipment. Services apply to vendor provided and government supplied equipment. Scope: This Performance Work Statement (PWS) defines the effort required for the delivery, setup, education installation, and management of Home Oxygen services and Ventilation, in accordance with applicable Joint Commission (JC) Home Care standards, to Veteran beneficiaries serviced by VAPHS . All content contained herein regarding the Joint Commission (JC) standards shall be interpreted in the following manner. Joint Commission standards or equal accrediting body that meets or exceeds the standards of the Joint Commissions is deemed acceptable. In other words, the Contractor is required to provide accreditations that meet or exceed the standards of the Joint Commission. All issuing and follow- up care provided under this contract shall be provided in accordance with all current Joint Commission standards. If the Contractor is re-surveyed by the Joint Commission during the contract term, the Contractor shall notify the Contracting Officer of survey dates, the outcome of the survey and provide copies of new certificates. Contractor s facility (ies) may be inspected by the VA Pittsburgh Healthcare System (VAPHS) prior to the contract award. This PWS includes the associated program management, human engineering, and logistic support planning requirements. The use of the terms beneficiary , Veteran , Patients , and Patient are used interchangeably and refer to the recipient of required supplies, equipment, and incidental services required under the contract. Period of Performance: The period of performance shall be one (6) month base period of performance with one (1) option period of six (6) months. THIS IS AN INTENT FOR SOLE SOURCE AND ANY RESPONSES WILL BE USED FOR MARKET RESEARCH ONLY.
