- This is a combined synopsis/solicitation for commercial items prepared in accordance with streamlined/simplified procedures under FAR Subpart 13.500 (c) and 12.6 to meet a crucial need for COVID-19 response.� This announcement constitutes the only solicitation; quotations are being requested. Solicitation number: 75R60221Q00072 This solicitation is issued as a request for quotation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-05.� Small Business Set Aside: No NAICS Code: 541611- Administrative Management and General Management Consulting Services Size Standard: $16.5M Product Service Code: R408 The purpose of this acquisition is to develop a framework for and to carry out project management and project planning for the implementation of Health Center COVID-19 Vaccine Program for participating HRSA-supported health centers and their clinical services delivery sites. The period of performance is 12 months from effective date of contract plus one (1) 12-month option period. INSTRUCTIONS TO OFFERORS: Data Universal Numbering System (DUNS) Number The Offeror shall enter, in the block with its name and address on the cover page of its offer, the annotation DUNS or DUNS+4 followed by the DUNS or DUNS+4 number that identifies the Offerors name and address. If the Offeror does not have a DUNS number, it should contact Dun and Bradstreet directly to obtain one. An Offeror within the United States may contact Dun and Bradstreet by calling 1-866-705-5711 or via the internet at http://www.dnb.com. An Offeror located outside the United States must contact the local Dun and Bradstreet office for a DUNS number. System for Award Management (SAM) Unless exempted by an addendum to this solicitation, by submission of an offer, the Offeror acknowledges the requirement that a prospective awardee shall be registered in the SAM database prior to award, during performance and through final payment of any contract resulting from this solicitation. If the Offeror does not become registered in the SAM database in the time prescribed by the Contracting Officer, the Contracting Officer will proceed to award to the next otherwise successful registered Offeror. Offerors may obtain information on registration and annual confirmation requirements via the internet at http://www.sam.gov or by calling 1-866-606-8220.� � The quote shall include price(s), a point of contact, name and phone number, business size, and payment terms. Quotes shall be submitted to Carolyn C. Taylor via email at ctaylor@hrsa.gov no later than 4:00 pm EST on March 4, 2021. �The response must clearly indicate the capability of the vendor to provide the exact items specified. Offerors responding to this solicitation are advised that, prior to award, the Government may request Offerors to submit additional information/data to support price.�Failure to submit the requested information may result in disqualification of the submitted quote. COVID-19 HHS reserves the right to exercise priorities and allocations authority with respect to this contract, to include rating this order in accordance with 45 CFR Part 101, Subpart A Health Resources Priorities and Allocations System. 52.212-2�Evaluation-Commercial Items�(Oct 2014) ������(a)�The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: EVALUATION�BASIS FOR AWARD The Government will make an award to the responsible Offeror whose offer represents the best value in terms of technical capability and price. All non-price factors (i.e., technical approach and past performance) are significantly more important than price. Best Value Determination: Determining the best value of the solicitation includes the following factors: Factor A: Technical Factor B: Past Performance Factor C: Price Each of the three (3) factors will be evaluated on a stand-alone basis. Factor A will be evaluated using points, Factor B will be evaluated based on confidence level, and Factor C will be evaluated on a total price basis. The Technical Factor is more important than the Price Factor. The Price Factor is more important than the Past Performance Factor. The Technical Factor and Past Performance Factor when combined are significantly more important than the Price Factor.
