Combined Synopsis-Solicitation for Commercial Items VHAPM Part 813.106 SAP: Soliciting Competition & Evaluation of Quotations SOP Original Date: 10/12/17 Revision 04 Date: 06/10/20 Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2021-03 (eff. 02/16/21). This solicitation is set-aside for Service Disabled Veteran Owned Small Business The associated North American Industrial Classification System (NAICS) code for this procurement is 339113 Surgical Appliance and Supplies Manufacturing, with a small business size standard of 750 employees. The FSC/PSC is 6515 Medical and Surgical Instruments, Equipment and Supplies. The NCO22G Department of Veterans Affairs, New Mexico VA Healthcare System, 1501 San Pedro SE, Bldg. 46, Albuquerque, MN 87108 is seeking to purchase a Arterion Power Injection System. The Surgical Service request purchase of the Medrad Mark 7 Arterion Injection System. The MEDRAD Mark 7 Arterion Injection System is the latest in the ""Mark"" series of angiographic injectors and includes variable flow capabilities with the MEDRAD VFlow Sterile Hand Controller, which provides the flexibility to deliver the accuracy of an injection system, thereby enabling consistent contrast delivery. We have the following technical specifications for the Arterion Injection System: This system is portable. This system is built to be compatible with the new siemens injection system. Color touchscreen display: All injection results are displayed on the main screen, store up to 35 injection protocols and review history of up to 50 actual injections. Ergonomic design: large handle on pedestal for mobility. Large smooth rolling casters lock in place. Articulating front load syringe: Quick one-handed tubing attachment, Twist & go disposable syringe. Clinical application service: on-site clinical training to help staff. Delivery shall be provided no later than 60 days after receipt of order (ARO). Delivery terms will be FOB destination. Quoters shall list exception(s) and rationale for the exception(s), if any. Submission of your response shall be received not later than 15:30 PM MST, 03/08/2021 at louis.wade@va.gov Quotes will only be accepted via email. Hand delivered or mailed quotes will not be accepted. Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212-1(f). No late quotes will be accepted. The FMS Vendor File Update form needs to be submitted in all cases to the Contracting Officer as part of the offer or proposal. Prior information sent to the VA Payment Center in Austin does not preclude submittal of the FMS Vendor File Update form. Failure to include this information could preclude the offeror from consideration for award or considerably delay award.
