- Purpose and Objectives: The intent of this Sources Sought Notice is to identify potential offerors capable of providing digitizing records. These are for the Lebanon VA Medical Center s CBOCs located within Pennsylvania. Responses to this Sources Sought Notice should demonstrate the firm's ability, capability, and responsibility to provide the principal components of supplies listed in the attached document. Responses should include the following information: Verify that the business is under 561621 of the North American Industry Classification System (NAICS), Verification of Size Standard under the NAICS, Business name, address, Point-of-Contact, if in possession of a Federal Supply Schedule contract, and if the business has verification as a Service Disabled Veteran Owned Small Business or Veteran Owned Small Business. All information is to be submitted via e-mail at Daniel.Johnson44@va.gov. Information provided will not be returned. All responses shall be in the English Language. Responses are due by 12:00 P.M. (EST) on Friday, March 5, 2021. No submissions will be accepted after this date and time. Questions can be submitted electronically to Daniel.Johnson44@va.gov. This is a Sources Sought Notice and submissions will be used for informational and planning purposes only. This notice does not constitute a formal Request for Quote (RFQ), nor is the government obligated to issue an RFQ. In addition, the Government does not intend to pay for any information provided under this notice. The Government is not obligated to notify respondents of the results of this survey. STATEMENT OF WORK DEPARTMENT OF VETERAN AFFAIRS COMMUNITY CLINIC VIDEO SURVEILLANCE, CCTV, INSTALLATION GENERAL INFORMATION 1. Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. Provide and install system components to effectively link together all video systems and current equipment for facility operations. 2. Background: The Lebanon VA Medical Center has a need to upgrade several areas of the Main Hospital. Specifically, the New ICU in Building 104, the new MOVE room in building 24 and the In-patient Psych Unit in Building 1. Along with the main Medical Center, the Lebanon VA Medical Center currently operates four (4) Outpatient Community Health Clinics in surrounding counties located in the southcentral region of Pennsylvania. Currently these sites have no CCTV security coverage to assist in the physical security of the identified facilities. The addition of CCTV coverage at each of these sites will enhance the safety and security of the facility site after hours of operations. The upgrades shall enhance the physical security of the identified areas along with protecting staff and patients. B. GENERAL REQUIREMENTS For every task, the contractor shall identify in writing all necessary subtasks (if any), associated costs by task, and together with associated milestone dates. The contractor s subtask structure shall be reflected in the proposal. All written deliverables shall be phrased in layperson language. Statistical and other technical terminology will not be used without providing a glossary of terms. Only submittals including 100% hardware, labor and software cost coverage shall be considered. C. ADDITIONAL SCOPE OF WORK The contractor shall establish an infrastructure to support CCTV operations at each site, install all components, hardware and system software. LEBANON VA MEDICAL CENTER: The contractor shall establish an infrastructure to support CCTV operations at each site, install all components, hardware and system software. All work for the main Medical Center will be conducted at 1700 South Lincoln Avenue, Lebanon, PA 17042. New ICU in B-104 Install 2 additional intercoms connected to the existing main entry door intercom. New MOVE Room in B-24 Install a PACS reader and configure the door to the space with electronic locking hardware. In-Patient Psych Unit in B-1 Install one (1) additional panic alarm in the locked vestibule, sally-port , of the Unit. Install remote door unlocking to the main door of the Unit. CBOCs: Lancaster Community Clinic Install 2 cameras inside the building to monitor the 2 entrance doors into the clinic. Located at 212 Willow Valley Lakes Dr., Ste. 208, Willow Street, PA 17584. Berks Community Clinic Install 4 cameras at the clinic. Two (2) cameras will monitor the front parking lot. One (1) camera will be installed to monitor the rear of the building. One (1) camera will be installed inside to the front entrance to the clinic. Located at the Ridgeview Professional Center, 2762 Century Blvd., Wyomissing, PA 19610. York Community Clinic Install one (1) camera inside the building to monitor the front entrance door to the clinic. Located at 2251 Eastern Blvd., York, PA 17402. Cumberland Community Clinic Install six (6) cameras at the clinic. Two (2) cameras will monitor the front parking lot. One (1) camera will monitor the side of the building. Two (2) cameras will monitor the rear parking lot. One (1) camera will be installed inside to monitor the front entrance to the clinic. Located at 5070 Ritter Rd., Mechanicsburg, PA 17055. D. MISCELANEOUS REQUIREMENTS Equipment List All equipment / products are listed in the project proposal. All installation, cabling and programming will be performed by Siemens Industry Inc. Installation Multiple areas require installation of deliverables and shall not distract from or cause insufficient delay or hazard the normal operations of the clinic, its staff, clients, visitors, etc... Equipment/System Testing All equipment will be tested by the contractor to ensure reliable operation of the system. Warranty All equipment shall come with standard one-year manufacturer s warranty. Upon completion of the year the system and components will be absorbed into the existing Siemens maintenance/service contract. E. CONTRACTOR RESPONSIBILITIES The contractor will be responsible for the actions of all individuals provided to work for the VA under this contract. In the event that damages arise for work performed by contractor provided personnel, under the auspices of this contract, the contractor will be responsible for all resources necessary to remedy the incident. Contractor will also be responsible for ensuring all provided personnel are properly licensed if operating motor vehicles on the facility and all personnel check in prior to start of the assigned contract and sign out with Police Services upon the completion of the contract. F. GOVERNMENT RESPONSIBILITIES The Government shall make available access to the specific sites so the contractor may accomplish all required work/tasks involved in the completion of this project. Inspection/Acceptation the government is required to inspect and accept equipment/system upon completion of the project and contractor certifying the system is operation and operates as it should. G. OTHER CONSIDERATIONS 1. Place of Performance: Lebanon VA Medical Center, Community Clinics locate in Lancaster County, Berks County, York County and Cumberland County Pennsylvania. 2. Hours of Operation: The services covered by this contract shall be furnished by the contractor as defined herein. The contractor will not be required, except in case of emergency, to furnish such services on a national holiday or during off duty hours as described below which the following meanings have: Work Hours Monday through Friday 08:00am to 4:30pm National Holidays: The ten (10) holidays observed by the Federal Government are as follows: New Year s Day Martin Luther King s Birthday Washington s Birthday Memorial Day Independence Day Labor Day Columbus Day Veterans Day Thanksgiving Christmas * Any other day specifically declared by the President of the United States to be a national holiday. Off Duty Hours: Monday through Friday, 4:30pm 8:00am, and all-day Saturday and Sunday. 3. Security: Lebanon VA Police Department shall provide Contractor access to specific sites as required. Work will be conducted during normal clinic operational hours.
