Page 1 of 4 STATEMENT OF NEED EYECON WORKSTATIONS BRAND-NAME ONLY LEBANON VA MEDICAL CENTER BACKGROUND The following is a Sources Sought/Request for Information notice. This posted notice is not a Pre-Solicitation or Solicitation document. The government is not currently soliciting bids, quotes or proposals at this time, however, reserves the right to do so at a later date. The Department of Veterans Affairs, Lebanon VA Medical Center, located at 1700 S. Lincoln Avenue, Lebanon, PA, has a requirement for four (4) Eyecon Workstations and controllers. This is a brand-name-only requirement. The NAICS Code is 339112. SCOPE OF NEED The contractor shall provide all resources necessary to accomplish the deliverables described in this Statement of Need (SON), except as may otherwise be specified and deliver to the desired location within the timeframe specified. GENERAL REQUIREMENT The Lebanon VA Medical Center has a brand-name-only requirement for four (4) Eyecon tabletop/capsule counting devices to enhance the accuracy of pill counting and the documentation of controlled substances. Eyecon and ScriptPro s Eyecon Interface will support accurate manual fills, double counts and physical inventory counts. The following items are required: 4 Eyecon Model #9420 4 Eyecon Interface Controllers Delivery and factory Installation (configuration, factory testing, and remote installation support) 1 Month of Support / Subscription DELIVERY All required items shall be delivered no later than thirty (30) calendar days after award. Delivery shall be coordinated with the Technical Point of Contact (POC). ** Note: In accordance with the U.S. Supreme Court decision regarding Kingdomware, service disabled veteran owned (SDVOSB) and veteran owned (VOSB), who are interested in this procurement and consider themselves to have the resources and capabilities necessary to provide these services shall be verified in VetBiz Registry as follows to be considered: http://www.vip.vetbiz.gov/. Process in 38 U.S.C. 8127(d) whereby a contracting officer of the Department shall award contracts on the basis of competition restricted to small business concerns owned and controlled by veterans, if the contracting officer has a reasonable expectation that two or more small business concerns owned and controlled by veterans will submit offers, and that the award can be made at a fair and reasonable price that offers the best value to the United States. For purposes of this VA specific rule, a service-disabled veteran-owned small business (SDVOSB) or a veteran-owned small business (VOSB), must meet the eligibility requirements in 38 U.S.C. 8127(e), (f) and VAAR subpart 819.7003 and be listed as verified in the Vendor Information Pages (VIP) database. GRAY MARKET ITEMS No used, refurbished, or remanufactured supplies or equipment/parts shall be provided. This procurement is for new Original Equipment Manufacturer (OEM) items only. No gray market items shall be provided. Gray market items are OEM goods intentionally or unintentionally sold outside an authorized sales territory or sold by non-authorized dealers in an authorized sales territory. No counterfeit supplies or equipment/parts shall be provided. Counterfeit items include unlawful or unauthorized reproductions, substitutions, or alterations that have been mismarked, misidentified, or otherwise misrepresented to be an authentic, unmodified item from the original manufacturer, or a source with the express written authority of the original manufacturer or current design activity, including an authorized aftermarket manufacturer. Unlawful or unauthorized substitutions include used items represented as new, or the false identification of grade, serial number, lot number, date code, or performance characteristics. Vendor shall be an OEM, authorized dealer, authorized distributor, or authorized reseller for the proposed equipment/system, verified by an authorization letter or other documents from the OEM or by an intermediary distributor authorized by the OEM. All software licensing, warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions. ADDITIONAL REQUIREMENTS Offerors shall provide with its response a letter from the OEM for any and all proposed products certifying that the Offeror or the Offeror s teaming partner, if acquiring the product through a teaming partner, is authorized to sell and provide the proposed product(s) as of the date of the submission of the quote/proposal. The OEM letter shall be provided on the OEM s letterhead and signed by an authorized representative of the OEM. Additionally, the Offeror s response shall explicitly self-certify that that it will not provide Gray Market Goods and/or Counterfeit Electronic Parts, and that it has the level of certification/specialization, as required by the OEM(s), to support authorized product sale and pricing for the products proposed. Failure to include the requisite OEM letter(s) and Offeror self-certification may render the Offeror s response unacceptable. Please provide a capability statement as well as any relevant literature addressing your organizations ability to supply the required items.
