Description
(i) This is a Combined Synopsis/Solicitation for the purchase of Mindray Accutorr Vital sign monitors, as prepared in accordance with the format in Federal Acquisition Regulations (FAR) Subpart 8.4, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. A firm-fixed price purchase order is anticipated. (ii) The solicitation number is 36C24421Q0398 and is issued as a request for quotation (RFQ). This solicitation is for a Brand Name Only Requirement. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2021-03 dated 1/14/2021. (iv) This solicitation is set aside 100% for Service Disabled Veteran Owned Small Businesses (SDVOSB) Set-Aside and the associated NAICS code 333912 has a small business size standard of 500 employees. (v) Statement of Work and Salient Characteristics Lebanon VAMC 1700 South Lincoln Ave. Lebanon, PA 17042 Title of Project: Purchase of additional Mindray Accutorr 7 Vital Signs Monitors for use by Nursing Services for inpatient care. Scope of Work: Vendor shall provide including, but not limited to portable, non-invasive blood pressure and pulse oximetry monitors and required accessories to include blood pressure cuffs, leads, cables and cords to ensure proper operation. Type of Contract: Firm Fixed Price General Requirement: The Lebanon VA Medical Center (LVAMC) has a need for seven replacement Mindray Accutorr 7 Vital Signs Monitors to be used by nursing staff in the inpatient sub-acute/Community Living Center units. The vital sign monitor will be used by nursing staff to assist with better outcomes for patient care by allowing vital signs to be monitored more frequently to ensure that cases of hypertension are caught at earlier stages. The Lebanon VA Medical Center currently has 110 Mindray Accutorr 7 Vital Signs Monitors in inventory and has found them to have the best features all around for better patient outcomes and employee use. Salient Characteristics: Large 8.4 display. Intuitive touchscreen. Manual, automatic and customizable modes. SPO2 Technology. Ability to provide manual input of vital signs to include respiration, pain level, glucose, fluids, and degree of consciousness. Rolling Stand with Cable Management Performance Period: Delivery shall occur within 30 days of receipt of contract Delivery: Delivery shall be made to the Lebanon VAMC located at 1700 S. Lincoln Ave., Lebanon, PA 17042; Attn: Warehouse. Biomedical Engineering will need to perform a safety check and enter the equipment into the system for annual Preventive Maintenance (PM) checks. Placement in the LVAMC shall be coordinated through the Contracting Officer Representative. No remanufacturers or gray market items will be acceptable. Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM. All software licensing, warranty, and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions. The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right to enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.
