Description
This Sources Sought Notice has removed requirements for Provider Transportation and Laboratory Transpiration. Vendors are instructed to use the following information and questionnaire to replace all previous questionnaires and information on this Sources Sought Notice. This is NOT a solicitation or request for quote (RFQ). This is NOT a request for proposal (RFP). This is NOT a job advertisement for hiring employees. The VA is seeking QUALIFIED VENDORS to provide a very brief response to this notice including only just what is asked for in this notice below. This is sources sought notice to industry and the only information is posted on Contract Opportunities web page at https://BETA.sam.gov/. Sources sought means seeking qualified vendors. The attachments included on the Contract Opportunities page do not include any additional information. If your company is interested please read this notice entirely and submit the required information, as requested herein. The Montana VA Health Care System is seeking vendors who have capabilities and interest in performing after-hours, ad hoc, on-call, transportation services for eligible veteran patients to/from Fort Harrison, MT. Services are needed 1600-0800, including weekends/public holidays, in these Montana counties: Lewis and Clark, Broadwater and Jefferson Montana. The eligible veteran patients are those who have received or will receive care at either the Montana VHA at Fort Harrison, the Sleep Center in Helena Montana, and Elk Mountain in Clancy MT. Transportation may also include to their place of residence from these locations. This is an on call service and can occur during inclement weather. For example, services could be required at 01:00 hours or on US Federal holidays, regardless of the weather. This VAMC is concerned with the risk of having an eligible veteran patient being stranded due to issues with contracted vehicles or contracted staffing. What pricing structure would you use/expect in a mutually agreeable contract? flat rate per mile and nothing else? OR Base rate within a set mile radius from the VAMC? Rate per mile beyond the set mile radius? Booking fees? No-show rates? Wait time rates? OR Expand on other rate categories or structures not listed above. Company s Recent Experience of Similar Size, Scope and Complexity to what is described in the notice. This can be US Federal contracted experience or experience performing services in the commercial sector. This can be your company s experience, your personal experience or even experience from your proposed subcontractor. Provide reference details, POC / client contact details, contact details of US Federal agency Contracting Officer (CO) or Contracting Officer Representative (COR), dollar values of services: location of services: start/end dates of services: Does your company currently have a similar service provision in this area? If not, how long would it take to become successfully established in the area to commence contracted services? Only written responses, to items 1 through 8 above, will be accepted at this stage. Do not submit additional documentation or attachments outside these requirements unless your company POC receives a direct request in writing by the Contract Specialist.
