Description
STRATEGIC SOURCING, BLOOD AND BLOOD PRODUCTS Purpose and Objectives: The intent of this Sources Sought Notice is to identify potential businesses especially any Small Businesses, Service Disabled Veteran Owned Small Businesses (SDVOSB) or Veteran Owned Small Businesses (VOSB) offerors capable of providing Blood and Blood Products as listed in the below Statement of Need utilizing NAICS Code 621991, Blood and Organ Banks. This product has the potential to be used throughout all VISN 4 Medical Centers listed below. Responses to this Sources Sought Notice should demonstrate the firm's ability, capability, and responsibility to provide the principal Blood and Blood Products listed below to all the VISN 4 VA Medical Centers listed below. Responses should include the following information: Business name, address, Point of Contact, Service Disabled Veteran Owned Small Business info and if they are a manufacturer of the items. All information is to be submitted via e-mail at ronald.kline@va.gov. Information provided will not be returned. All responses shall be in the English Language. Responses are due by 12:00 pm (EST) on Thursday, March 18, 2021. No submissions will be accepted after this date and time. Questions can be submitted electronically to ronald.kline@va.gov. This is a Sources Sought Notice and submissions will be used for informational and planning purposes only. This notice does not constitute a formal Request for Quote (RFQ), nor is the government obligated to issue an RFQ. In addition, the Government does not intend to pay for any information provided under this notice. The Government is not obligated to notify respondents of the results of this survey either. PURPOSE: The contractor shall be able to supply Blood/ Blood Products for all VISN 4 VA Medical Centers listed above. They will need to provide Reference Laboratory services for any additional patient testing for difficult antibody workup or problems with incompatible blood with help in finding antigen negative units. II. REQUIREMENTS: 1. The Contractor must meet the standards of all licensing and accreditation agencies to include the following: College of American Pathologist (CAP), Occupational Safety and Health Administration (OSHA), Food & Drug Administration (FDA), and American Association of Blood Banks (AABB). 2. The Contractor shall provide directed donation service specifically for the needs provided a Directed Donation Blood Prescription when submitted by the requesting physician. The units drawn shall be separated into component parts; the component ordered shall be tagged for identification and shipped to the VISN 4 Medical Centers and upon completion of testing provided it is compatible with the patient. 4. The Contractor shall service autologous donors provided an Autologous Blood Prescription by the requesting physician and the donor meets the blood supplier standards for autologous donors. The units of blood drawn shall be tested and sent to VISN 4 Medical Centers upon completion of testing. 5. The Contractor shall provide Patient Immunohematology Reference Testing to include any special testing that may be needed. Such testing would be for identification of Antibodies, Compatibility Testing, antigen negative units when indicated. Testing may not be limited to this. 6. The contractor will abide by all security and integrity of the patient information per the HIPAA regulations. 7. The Supplier will provide set-up a schedule for routine deliveries. III. PROCEDURES 1. Blood Products ordered shall set a routine delivery schedule that will be coordinated with the site. 2. The government shall receive full credit for expired red cells or any blood product that is found to be unsuitable for use. 3. The contractor shall accept any blood unsuitable for Transfusion, Recall, Withdrawals and Transfers. IV. PERFORMANCE STANDARDS Description Para ref Standard Routine Delivery III,1. All recalls and non-conforming notifications will be provided as deemed y the accreditation service 100% NARA Records Management Language for Contracts Required The following standard items relate to records generated in executing the contract and should be included in a typical Electronic Information Systems (EIS) procurement contract: 1. Citations to pertinent laws, codes and regulations such as 44 U.S.C chapters 21, 29, 31 and 33; Freedom of Information Act (5 U.S.C. 552); Privacy Act (5 U.S.C. 552a); 36 CFR Part 1222 and Part 1228. 2. Contractor shall treat all deliverables under the contract as the property of the U.S. Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest. 3. Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records. 4. Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act. 5. Contractor shall not create or maintain any records containing any Government Agency records that are not specifically tied to or authorized by the contract. 6. The Government Agency owns the rights to all data/records produced as part of this contract. 7. The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Contractor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data. 8. Contractor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.] or state of completion [draft, final, etc.]. 9. No disposition of documents will be allowed without the prior written consent of the Contracting Officer. The Agency and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schedules. 10. Contractor is required to obtain the Contracting Officer's approval prior to engaging in any contractual relationship (sub-contractor) in support of this contract requiring the disclosure of information, documentary material and/or records generated under, or relating to, this contract. The Contractor (and any sub-contractor) is required to abide by Government and Agency guidance for protecting sensitive and proprietary information.
