Description
STATEMENT OF WORK Renovate Basement, Building 1 VHA Northern Indiana Health Care System Ft. Wayne, IN Project: 610A4-19-104 Solicitation will be posted to beta.SAM.gov on or around March 11, 2021 Project Background: The basement has not been renovated for decades, if at all, and is being renovated in this project to modernize the space. Statement of Work: Contractor will provide all labor, equipment, materials, and supervision necessary to Renovate Basement, building 1 at the Ft. Wayne campus of the VHA Northern Indiana Health Care System to include but not limited to the following: Renovate space in the basement for Environmental Management Services, Biomed, Pharmacy, Mailroom, Engineering Services, Office of Information and Technology, Graduate Medical Education, and Medical Imaging by following the developed design and specifications. Campus buildings are pre-1950 construction; asbestos containing material and lead based paint are present in the buildings and must be removed and handled per OSHA guidelines. VA shall provide most recent surveys of hazardous material. The design identifies hazardous locations and materials as applicable to complete the project. The project is divided in three phases per the A/E s drawings. As a working Hospital the adjacent spaces and functions will need to be maintained and a high degree of coordination, scheduling and phasing will be required to allow the continuation of the existing Hospital Services during the entire Construction Project. Each phase must have a punch list and a 30-day work stoppage between phases at no additional cost to the government to allow for internal moves. A Notice to Proceed (NTP) will be issued before the next phase may begin. Phase I should take 300 days and the work will be conducted in rooms 15-26 and 45, 45A, & 45B. Phase II should take 210 days and the work will be conducted in rooms 49A-59A and the east basement corridor. Phase III should take 270 days and the work will be conducted in rooms 30-35 and 43, 44, & 44A and the west basement corridor. All work to be completed at the VA Northern Indiana Health Care Ft Wayne Campus, 2121 Lake Ave, Ft Wayne, IN 46805. Justification of Work: This project is designed to transform the VA s business operations by modernizing systems and focusing resources more efficiently to be competitive and to provide world-class customer service to Veterans and its employees by; streamlining workflow, optimizing storage space, providing real-time electronic inventory tracking, and medication barcode verification to prevent dispensing errors at the Ft Wayne VA.
