DESCRIPTION: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation Number 36C25021Q0455 is being issued as a Request for Quote (RFQ) and the intent is to award a firm fixed price for Semi-Annual, Annual, 30 Day Follow Up inspections, and Full Load Tests on the 18 Traction Elevators and 12 Hydraulic Elevators. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26.  This requirement is a Service-Disabled Veteran Owned Small Business set-aside for commercial items under the North Atlantic Industrial Classification System (NAICS) 541350. REQUIREMENTS: The Louis Stokes Cleveland Department of Veterans Affairs Medical Center (10701 East Blvd Cleveland, Ohio 44106) is seeking potential contractors to provide Semi-Annual, Annual, 30 Day Follow Up inspections, and a Five (5) Year Full Load Tests on the 18 Traction Elevators and 12 Hydraulic Elevators located at the Cleveland VA Medical Center (10701 East Blvd, Cleveland, OH 44106). The contract will be for a base year with four one-year options. PROVISIONS AND CLAUSES: This combined synopsis lists one or more clauses by reference with the same force and effect as if they were given in full text (FAR 52.252-2). The provisions at FAR 52.212-1, Instructions to Offerors-Commercial Items, apply to this acquisition with the following addendum to the provisions of the standard provisions. In accordance with Federal Acquisition Regulation FAR 52.212-2 Evaluation-Commercial Items all offers will be considered under Comparative Analysis. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers. (1) Technical capability. Additionally, any potential offerors quoting a brand equivalent must supply supporting documentation that the product quoted is of equal or greater technical capability matching the specified equipment in features, quality, warranty terms, compatibility with existing systems, and functionality to be considered. (2) Price: Quote for each line item, including but not limited to the requirements listed above. NOTE: The Government reserves the right to award without discussions, vendors shall submit the most favorable terms. PERFORMANCE WORK STATEMENT Scope. To provide Semi-Annual, Annual, 30 Day Follow Up inspections, and Five (5) Year Full Load Tests in year 2025, on the 17 Traction Elevators and 13 Hydraulic Elevators located at the Wade Park VA Medical Facility. Inspections will take place in May and November. 30-day follow-ups will take place in June and December (please know the identified inspection and follow-up dates are subject to change.) In the event a Traction Elevator and/or a Hydraulic Elevator is added or removed from the above 30 elevators, the inspection price will be adjusted per the contract agreement. In addition to the on-site inspections, tests, and follow up inspections, vendor will provide one (1) electronic copy and two (2) paper copies (1 bound and 1 unbound) as well as two (2) electronic CD of each inspection. Items noted in the Narrative Report are color coded as follows: Items noted in Black are typically the owner s responsibility. Items noted in Red are typically the Maintenance Provider s responsibility. The bound copy will be sent to the Louis Stokes VA Medical Center 10701 East Blvd. Cleveland, Ohio 44106 Attn: Electrical Shop Supervisor BE-440 The other copy is to be sent directly to the elevator company servicing the Wade Park VA Medical Center. The electrical shop supervisor will provide point of contact information for the elevator company to the vendor. The elevator company servicing the Louis Stokes VAMC is to be notified by the vendor that they have 30 days upon receipt of inspection documentation to perform corrective action. A re-inspection will then take place by the inspection vendor after the 30-day limit has been reached. All coordination between the vendor and the elevator company pertaining to the elevator inspections is the sole responsibility of the inspection vendor. The inspection vendor may be requested to write additional reports pertaining to the elevator inspection if the elevator company is not cooperative with making the necessary corrections. Inspection vendor is to perform re-inspection and then send the results of the punch list to the Electric Shop via email. All outstanding violations pertaining to the follow up elevator inspection will be sent to the contracting officer for corrective action by electrical shop supervisor. This Service Contract is a direct result of elevator violations that where missed and may not have been corrected in the past. This contract will provide the contracting officer an expert in the field of elevators which will act as a third party along with the electric shop as a basis to build a case upon if necessary. The customer will be responsible for all other inspection violations that do not fall under the elevator service contract. Safety Requirements. Safety Related Issues: Safety of the vendors personnel is the vendors responsibility. The VA will not intervene except when the safety of VA personnel or property is at risk. Accident Prevention (FAR 52.236-13). Onus is on contractor to safeguard the public, contractor personnel, etc. during inspections. Provide appropriate signage, barricades, etc., as appropriate and as directed by Contracting Officer. Contractor will dictate appropriate PPE for each work area and indicate clearly those requirements at all entrances to areas (VA issue sign to maintain). Vendor is to notify the electrical shop supervisor 60 days prior to schedule the initial inspections and 30 days for the follow up inspections. Security: Ensure all elevator machine room doors are always locked. Contractor is also responsible for security badges of all contractor/subcontractor personnel. Every worker must have a badge and form of identification when working on VA property, and is subject to VA security checks and enforcement actions while working on VA property. Smoking and consumption of Alcohol on VA property: No smoking in any building on station. Provide contractors with a copy of Medical Center Directive 1085. Mechanical rooms and contractor occupied areas are no exception. Also, contractors are NOT allowed to bring alcohol on to government property. Those who violate this policy can face up to a $300 fine. Use of Existing VA Canteen: Contractors may use VA canteen for purchasing food so long as they have appropriate ID badge worn visibly above the waist and footwear are free of excess dirt and debris. Above Ceiling Work Permits: Contractor shall request and follow all provisions of approved permit(s) while working in the Medical Center. Working outside of construction barriers without having approved permit with construction personnel is cause for immediate cessation of work by the COTR/ACO. Lock Out/Tag Out Energy Control Program shall be observed by all contractor personnel. Contractor is to submit a copy of their policy for approval. Provide contractors with copy of Medical Center Lock Out/Tag Out Policy 138-031. Professional Behavior: Please remind all staff and subcontractors that they are working in an active medical center environment, and professional behavior is expected at all times. Since work is sometimes performed directly adjacent to active offices or patient care areas, please be cognizant of excessive noise and disruptions to those nearby (radios too loud, use of foul/inappropriate language, etc.) All inspections to be completed immediately to bring the Wade Park VAMC up to code and the latest safety standards from the day the contract is signed. The quote will include all labor, material, equipment, software, freight charges. Inspections will be scheduled with the VA. The Main Facility Service Elevators and single use elevators are to be done on off hours and can be coordinated with VA to meet schedules. The most practical time for these elevators is on 2nd shift 3PM to 11PM. When the scope of work is completed the vendors shall brief the electrical shop supervisor on all violations. PURPOSE This Quality Assurance Surveillance Plan (QASP) provides a systematic method to evaluate performance for the stated contract. This QASP explains the following: What will be monitored. How monitoring will take place. Who will conduct the monitoring. How monitoring efforts and results will be documented. This QASP does not detail how the contractor accomplishes the work. Rather, the QASP is created with the premise that the contractor is responsible for management and quality control actions to meet the terms of the contract. It is the Government s responsibility to be objective, fair, and consistent in evaluating performance. This QASP is a living document and the Government may review and revise it on a regular basis. However, the Government shall coordinate changes with the contractor. Copies of the original QASP and revisions shall be provided to the contractor and Government officials implementing surveillance activities. 2. GOVERNMENT ROLES AND RESPONSIBILITIES The following personnel shall oversee and coordinate surveillance activities. a. Contracting Officer (CO) - The CO shall ensure performance of all necessary actions for effective contracting, ensure compliance with the contract terms, and shall safeguard the interests of the United States in the contractual relationship. The CO shall also assure that the contractor receives impartial, fair, and equitable treatment under this contract. The CO is ultimately responsible for the final determination of the adequacy of the contractor s performance. The Government performs surveillance to determine if the contractor exceeds, meets or does not meet these standards. The Performance Requirements Summary Matrix, paragraph in the Performance Work Statement (PWS), includes performance standards. The Government shall use these standards to determine contractor performance and shall compare contractor performance to the Acceptable Quality Level (AQL). The government reserves the right to monitor services in accordance with Performance Based Matrix. Monitoring of contractor s performance shall be done by the Contracting Officer Representative (COR): Contract noncompliance: Incidents of contractor noncompliance as evidenced by the monitoring procedures shall be forwarded immediately to the Contracting Officer. A Quality Assurance Surveillance Plan will be signed by the Contractor Program Manager and COR, outlining the evaluation criteria to ensure the contractor is aware of the management and quality criteria required to meet the terms of the contract. Frequency of Measurement: During contract performance, the COR will periodically analyze whether the negotiated frequency of surveillance is appropriate for the work being performed. Semi-annual (or quarterly if warranted) reports are prepared by the COR. Frequency of Performance Assessment Meetings: The COR shall visit with the contractor either on an as-needed basis or, at a minimum, twice during the Period of Performance: The first time during the middle of contract period and the second time will be towards the end of contract period to assess performance and shall provide a written assessment. This written assessment will be forwarded to the Contracting Officer as documentation of contractor s performance and/or for a request to exercise option period, renewal of a contract, or termination of a contract. Contractor reviews: Contractor will review semi-annual (or quarterly) performance measure reports and acknowledge receipt of report by returning a signed copy, along with any comments, to the Contracting Officer within 30 days of receipt of the report.
