- COMBINED SYNOPSIS/SOLICIATION �COMBO�: Combat Medical Training i. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. ii. Solicitation FA487721QA107 is issued as a Request for Quotation (RFQ). iii. This solicitation document incorporates provisions and clauses of those in effect through Federal Acquisition Circular 2021-03 effective 16 February 2021 , Defense Procurement Notice 20210115, and Air Force Acquisition Circular 2019-1001 effective 1 October 2019 iv. This government is soliciting under full and open competition. The associated North American Industrial Classification System (NAICS) code for this procurement is 611699 with a small business size standard of $11,000,000.00 average annual revenue. v. Davis Monthan Air Force Base intends to award a firm-fixed price contract for the following: �Product Description� a. CLIN 0001: Combat Medic Training QTY: 1 UI: EA b. Please refer to the Performance Work Statement in the attachments for specific training requirements. vi. Training shall be conducted in accordance with the Performance Work Statement vii. Offeror must provide confirmation from the medical school, as well as providing their respective certification that the place of performance is directly managed and audited by a co-located and accredited ( Liason Committee on Medical Education / LCME) U.S. graduate medical school program. viii. We will evaluate all responsive quotes, final selection will be based on lowest-priced technically acceptable. Technical acceptability is defined and referred to within this solicitation document as the offeror�s capability statement to meet the defined performance characteristics of the training and confirmation that the place of performance is directly managed and audited by a colocated and accredited (Liason Committee on Medical Education / LCME) U.S. graduate medical school program. ix. Important Dates/Times (All Times are Arizona Local Time) a. Questions are due by 11�March 2021, 2:00 PM. (NO questions will be considered after the questions due date.) b. Answers will be provided by�12 March�2021, 2:00 PM. c. All quotes shall be submitted by 15�March 2021, 2:00 PM x. It is the responsibility of the offeror to review the posting for any changes or amendments that may occur concerning this RFQ. It is the responsibility of the offeror to ensure all quotes and/or questions are submitted in a timely manner by specified due date. xi. Solicitation Documents attached are as follows. a.
