Page 2 of 3 THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR QUOTE. The NCO 12, Great Lakes Acquisition Center, 115 S. 84th Street, Suite 101, Milwaukee, WI 53214, is performing market research to determine if there is a sufficient number of qualified: (1) Service Disabled Veteran Owned Small Business (SDVOSB); (2) Veteran Owned Small Business (VOSB); or (3) Small or emerging small business firms, or (4) Other Than Small business firms, to provide Parks Flo Lab 2100-SX2 or Equal for the Tomah VAMC. General Requirements: Brand Name or Equal: Parks Flo Lab 2100-SX2 Compatible with AGFA PACS Compatible with VISTA and CPRS 1. Base system requirements: a. Dual Channel Pulse Volume Recording/pneumoplethysmography (necessary for same time comparison) b. Dual Channel Photoplethysmography (as for PVR- bilateral limb comparison must be in an equal state post exercise, rest, etc.) c. AC/DC coupled PPG sensors (not all manufacturers offer AC/DC sensors in the same sensor, which is problematic if you are trying to put a blood pressure cuff, and 2 sensors on a diseased toe) d. Doppler continuous wave pencil probes, high frequency 8 MHZ, and low frequency 4 MHZ. 2. Computer Components a. Reporting interface can be performed using the mouse, keyboard or wireless remote b. Capabilities for PDF, DICOM, and XML c. Full page reports with waveforms as required for Medicare reimbursement. 3. Power Supply a. Cart shall have power supply sufficient to power computer and all factory installed accessories b. All factory installed accessories requiring power must be compatible with power supply and connections no systems where the main computer module is powered through a different plug from the equipment used on the patient. c. Power cord shall be no less than 6 feet 4. Cuff Inflator a. Full-Function Remote Control for Automatic & Manual Inflation and Deflation, Capture, Forward, Back, scrolling of the waveform, and Accept. b. 10-Cuff Simultaneous Hookup c. Two speed inflator d. Maximum inflate pressure of 300 mmHg e. BP memory with auto ABI calculation 5. Customer Service/Support a. Warranty coverage shall include on-site repairs b. Warranty coverage shall include product replacement during repairs c. Vendor shall provide notification to purchasers of manufacturer identified problems d. Replacement parts shall be shipped within 48-hours from time customer service receives notification of needed part not all vendors supply this; resulting in down time for patient care. A capability statement that addresses the organizations qualifications and ability to provide the requirement depicted above. Documentation from manufacturer (i.e. correspondence from manufacturer) of your firm being an authorized distributor for the manufacturer.
