Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation: W50S8Z-21-Q-0009 is being issued as a Request for quote (RFQ). All solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2021-05. This procurement is a 100% Small Business set-aside IAW FAR 19.502-2(a). The NAICS code is 713940 and the small business size standard is $22.0 Million. The following commercial items are requested in this solicitation: CLIN 0001 - 6 Months - Fitness Program Services� Maximum of 10 participants �� See attached PWS CLIN 0002 (Option) - 6 Months - Fitness Program Services� Maximum of 10 participants � �� See attached PWS� Quotes Submission: Quotes should be submitted by email (preferred method) to the Contracting Office at jason.rice.27@us.af.mil no later than Friday April 2nd at 03:00 PM PST. Quotes shall be clearly marked RFQ W50S8Z-21-Q-0009. Offerors are hereby notified that if their quotes are not received by the date, time, and location specified in this announcement, it will not be considered by the agency. It is the responsibility of the offeror to view the website for any changes or amendments to this RFQ. Quote per line item format as follows: CLIN 0001 � 6 Months - Fitness Program Services �� Price per member $______________ X 10 = $______________ CLIN 0002 � Option 1; 6 Months - Fitness Program Services�� Price per member $______________ X 10 = $______________ Requirements for your quote: Please submit a quote on your company letterhead showing a price per contract line item (CLIN) with the total price at the bottom. Include the following information on your quote: Cage code, Tax ID number, company name, phone number, contact, and email. The award will only be made to an offeror that is all-inclusive of the requirements on this solicitation. One Contract will be awarded based on the following evaluation factors that are deemed the best value to the government. Failure to identify inside your quote that you meet all technical aspects of the PWS will make your quote ineligible for award. All quotes determined to be responsible will be evaluated by a government technical representative. The award for this will be made without discussions. No Site Visit is authorized at this time. - Technical capability of the services offered. - Price Quotes must be Firm Fixed Priced Award(s) will be made to the most technically capable offeror meeting all requirements according to the PWS that meets the minimum performance standards and is determined responsible in accordance with FAR Part 9. Ensure your quote is detailed enough to determine that it meets that terms and conditions of the requirement and the performance standards in the PWS. The following provisions and clauses apply to this acquisition: 52.217-9 Option to Extend the Term of the Contract. As prescribed in 17.208(g), insert a clause substantially the same as the following: Option to Extend the Term of the Contract (Mar 2000) � � � (a) The Government may extend the term of this contract by written notice to the Contractor within 200 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 30 days before the contract expires. The preliminary notice does not commit the Government to an extension. � � � (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. � � � (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 12 Months.
