- Solicitation Amendments: 01 - Dated 12 MAR 21, Extend RFQ response date to 25 MAR 21.� ------ This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation Number FA441721QC006 is hereby issued as a Request for Quote. Response Date: See betaSAM Contract Opportunities This combined synopsis/solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2021-04, Effective 19 January 2021. This acquisition is set aside 100% for small business concerns. The North American Industry Classification System (NAICS) code for this project is 334220. The Product Service Code (PSC) is 5820. The Size Standard for this NAICS is 1,250 employees.� This acquisition is conducted IAW FAR Part 13 Simplified Acquisition Procedures. Evaluation of commercial items in accordance with FAR 52.212-2 will be utilized using lowest price technically acceptable. Site Visit:� - A site visit has been scheduled for the solicitation, FA441721QC006 MDG Distributed Antenna System (DAS) Installation.� PLEASE NOTE: DUE TO COVID MAX ATTENDANCE ALLOWED PER SITE VISIT IS SIX (6) PERSONNEL AND MASKS ARE REQUIRED AT ALL TIMES WHILE ON THE INSTALLATION. With limited site visit attendees the government has set up two (2) time slots on 10 March 2021, 2:00 PM (CST) and 3:00 PM (CST).� If attendance does not go past the six (6) personnel limit, then the government will only have one (1) site visit at 2:00 PM (CST).� Email written request with Full Name and Company of those who will attend the site visit to Laura Kattner at laura.kattner.1@us.af.mil and Jesse Capik at jesse.capik@us.af.mil. Requests must be received NLT 5 March 2021, 3:00 PM (CST). Once the official request to attend the site visit is received, the physical location and address shall be provided to the requesting party. ALL COMMERCIAL VECHICLES WILL ENTER THROUGH THE COMMERCIAL VISITOR INSPECTION (CVI) GATE LOCATED OFF OF MARTIN LUTHER KING JR BLVD. INDIVIDUALS IN PERSONAL OWNED VEHICLES (POV) WILL USE THE MAIN GATE OFF OF HIGHWAY 98. INDIVIDUALS IN POV�S WILL NEED TO OBTAIN BASE PASS FROM THE VISITOR CONTROL CENTER (VCC) ON THE SOUTHSIDE OF HWY 98. PLEASE ALLOW EXTRA TIME TO OBTAIN YOUR PASS. - SUBMISSION OF QUESTIONS: Questions regarding this solicitation and site visit must be submitted in writing in Word Format to the individuals listed above no later than the close of business 2 business days after the site visit. No questions shall be answered during the site visit. ALL QUESTIONS SHALL BE SUBMITTED IN WRITING.� - VALID ID: For either the site visit or submission of proposals, all personnel requesting access to Hurlburt Field must possess a valid State or Government picture identification card. Personnel requesting vehicle access to Hurlburt Field must provide a valid driver�s license, current vehicle registration, and valid vehicle insurance certificate. The contractor shall provide all labor, equipment, materials, tools, parts, shipping, and all other items necessary to deliver and install the brand name or equal items Distributed Antenna System in accordance with the attached RFQ FA441721QC006, Statement of Work and Salient Characteristics.
