Description
Statement of Work Enhanced Entry Screening Services Background: Due to the recent spread of COVID-19 in the state of Colorado, the City and County of Denver has ordered the VA Office of Community Care to screen all VA Government employees at the beginning of their shift to include the taking of the employees� temperature. Scope of Work: The primary purpose of this contract is to provide a medical professional capable of administering temperature screening and tallying each screen for roughly 100 employees working within the facility. In addition, at a minimum, this requires one Certified Nursing Assistant (CNA) to provide COVID-19 screening for the Office of Community Care at 3773 Cherry Creek Drive, Denver, Colorado from 5:45am-9:45am, Monday through Friday, excluding federal holidays. CNA must be able to operate a no contact scanning thermometer, ask COVID-19 screening questions, and record the number of VA employees screened during the work period. CNA will inform any employee who does not meet screening criteria that the employee must return home and report findings to supervisor and record employees name and department for notification of leadership for follow up. CNA must be familiar with the proper PPE protocols and be able to wear a protective mask, gloves, and face shield when available. CNA must be articulate and able to explain COVID-19 protection measures to employees when asked and explain to employees that the masks available are to be used for an entire week to the maximum extent possible. Hours may change as necessary to meet mission needs. Requirements: The contractor shall provide Enhanced Entry Screener(s) that meet the minimum requirement: Possess at minimum a High School education or equivalent. Be literate in English and able to read and understand printed regulations, detailed written orders, and training instructions. Be able to communicate effectively and resolve conflicts in a civil and professional manner. Be able to successfully pass the Enhanced Entry Screening prior to starting each shift. Must be currently certified as a CNA (Certified Nursing Assistant). Mandatory Tasks: The contractor shall provide personnel who shall be capable and provide the following services listed: Taking temperatures with no contact thermometers. Asking questions related to COVID-19 risk, including, at a minimum, the following: Are you experiencing any symptoms of COVID-19? The symptoms are cough, feeling feverish, and/or difficulty breathing. Within the last 14 days have you been in close physical contact (meaning with 6 ft or closer) with a person you know to have laboratory-confirmed COVID-19? For purposes of this question, an active case of laboratory-confirmed COVID-19 means that a healthcare professional confirmed that the person had COVID-19 through laboratory testing, and that your contact with the person occurred within 14 days of the person receiving confirmation the he or she had COVID-19. In the last 14 days, have you received instructions from a public health authority to self- observe, self-isolate, or self-quarantine? An affirmative response to any question, or a temperature of 100.4F or above, would bar an employee, contractor, or visitor access to a federal facility. Additional screening questions may be added however the program office SME (Subject Matter Expert) or program office personnel appointed as the point of contact (POC) for this contract is responsible for staying updated on COVID-19 screening procedures. Thus, the program office SME or POC is responsible for updating the contracting office and Contracting Officer (CO) to request a modification to the contract if additional screening questions, procedures needs to be added. The Contractor shall provide all labor, tools, supervision and supplies (i.e. includes PPE for their employee) to perform Enhanced Entry Screening Services. At a minimum Enhanced Entry Screeners shall wear the following Personal Protective Equipment (PPE): Masks Eye Protection Gloves Contractor shall provide service in accordance with this SOW for the following hours listed below in table 1.0 and for the full duration of each period of performance. Table 1.0 Number of Screeners Days Tour of Duty Total Shift Hours 1 Monday thru Friday 5:45 AM to 9:45AM 4 Additional Services: If additional services are required outside of the hours of coverage specified herein, the Contractor must receive prior written approval from the Contracting Officer (CO) via contract modification. Any additional services performed by the Contractor without this authorization will be performed at no additional cost to the Government. Per hour prices for such additional services shall be at the same rate as prices charged in the contract during each respective period of performance. If an emergency should arise, a verbal approval can be obtained from the CO, however, the Contractor must submit a written quote for such additional work to the program office COR. The COR is responsible for submitting a proper package along with the contractors quote in accordance with NCO-19 emergency procedures. If accepted, a modification will be issued to the Contractor and the contract price shall be adjusted accordingly. The following holidays are observed by the Federal Government: New Year�s Day January 1st Martin Luther King�s Birthday Third Monday in January President�s Day Third Monday in February Memorial Day Last Monday in May Independence Day July 4th Labor Day First Monday in September Columbus Day Second Monday in October Veterans Day November 11th Thanksgiving Day 4th Thursday in November Christmas Day December 25th Any other day specifically declared by the President of the United States to be a federal holiday. When one of the holidays falls on Sunday, the following Monday shall be observed as a Federal Holiday. When a holiday falls on a Saturday, the preceding Friday shall be observed as a Federal Holiday. Period of Performance: The period of performance shall consist on one twelve-month base year and two twelve-month option year periods. Quality Control: The selected contractor shall develop and maintain an effective quality control program to ensure services are performed in accordance to this PWS. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The contractor�s quality control program is how the contractor ensures the work complies with the requirement of the contract. The Contractor shall be responsible for quality control; (QC) for all work accomplished during the performance of this contract. Work and documentation produced by the selected contractor may be regularly reviewed by the POC/SME TBD.
