Description

AMENDMENT 0002: Changes have been made to the scope of work-see Amendment 0002_Attach 1_SOW. Also, the site visit log has been added as an attachment. See the SF-30. The solicitation response date has been changed to Friday, March 19, 2021, 5:00pm PST. AMENDMENT 0001: Add to the scope of work-spec for color/shade. See the SF-30 and Attachment (Amended SOW.) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-94, 2005-95. It is the contractor's responsibility to be familiar with applicable clauses and provisions. Clauses and provisions can be found at https://www.acquisition.gov/far/52. The solicitation number is 140P8321Q0015 and the solicitation is issued as a Request for Quotation (RFQ). The NAICS code is 238330, Flooring Installation with an associated small business size standard of $16.5 million. This solicitation is a 100% total small business set-aside, and will result in a firm fixed price contract to a single vendor determined to be responsive and responsible. The quoter must be registered in the System for Award Management (SAM) website: https://www.sam.gov/ with the NAICS code 238330. DESCRIPTION. Contractor shall remove and replace all carpeting as noted in the Scope of Work for the National Park Service (NPS), Whitman Mission Historical Park, Visitor Center (VC) in Walla Walla, Washington. PERIOD OF PERFORMANCE (POP). Performance of this contract will begin 30 days after receiving award. Estimate Period of Performance for service: April 19, 2021 to May 21, 2021. The Contracting Officer�s Representative (COR) will schedule with the contractor the start date and time. ATTACHMENTS FOR THIS REQUEST FOR QUOTE (RFQ). There are a total of 3 attachments: The SF-18 and Terms and Conditions; Attachment 1-Scope of Work; Attachment 2-Dept of Labor Wage Determinations; Attachment 3-Submittal Form. QUESTIONS REGARDING THIS REQUEST FOR QUOTE (RFQ) must be submitted in writing via email (phone or fax questions will not be accepted) 5 days before responses are due. Send email questions to Nadine_Joe@nps.gov and reference the RFQ number in the subject line. WHAT AND WHEN TO SUBMIT - - CONTENTS OF QUOTE: Examination of Site: Before submitting a quote, each quoter is encouraged to have examined the site to satisfy the existing conditions under which they have pledged to operate or that in any manner might affect the work under this Contract. A site visit will be on Thursday, 03/11/2021. Site visits will need to be scheduled with the Contracting Officer�s Representative, (COR), Trent Martinez at 208-843-7055, trentin_martinez@nps.gov. Note, certain time slots will be made for each contractor due to COVID-19 restrictions. All contractor�s will be required to wear a mask in federal buildings and follow local county restrictions. Submit the following to Nadine_Joe@nps.gov by Wednesday, March 17, 2021, 5:00 pm Pacific Standard Time (PS): -Complete box 11b (DUNS#, Email Address, Overall Total) and boxes13-16 of the SF-18; -Fill in and complete in pricing in paragraph 05 and on the SF-18.. -Acknowledge any amendments issued IF APPLICABLE; -Provide all documentation required in paragraph 19 of the terms and conditions, Evaluation Factors, for NPS to conduct evaluations. A) On the SF-18, Provide Brand of Carpet and Trim. Provide the specifications of the items quoted; B) Provide 2 completed projects for experience and all information noted. -Submit either the filled out reps/certs provision 52.212-3 manually or note that you have completed the representations and certifications in SAM. HOW TO SUBMIT QUOTE: Submit quote by email by the response date and time. Emailed quotes will need to be stamped received before the response date and time. Hand delivered, telephone or faxed quotes will not be accepted. Email to Nadine_Joe@nps.gov and include the RFQ number 140P8321Q00015 in the subject line of the email.
