Description

The following are modifications being made to subject QSL Announcement: Required Item 5.76 SUTURE, ABSORBABLE 2-0 BRAIDED SYNTHETIC (ABSORPTION RATE 40-50 DAYS) UNDYED 36 IN/90 CM LENGTH SUTURE WITH TAPER POINT 1/2 CIRCLE 36 MM-37 MM LENGTH NEEDLE, 1 SUTURE PER PACK is now Optional Item 26. Required Item 5.108 SUTURE, ABSORBABLE 3-0 BRAIDED SYNTHETIC (ABSORPTION RATE 40-50 DAYS) UNDYED 36 IN/90 CM LENGTH SUTURE WITH TAPER POINT 1/2 CIRCLE 36 MM-37 MM LENGTH NEEDLE, 1 SUTURE PER PACK is now Optional Item 30. Required Item 5.157 SUTURE, NON-ABSORBABLE 2 BRAIDED POLYESTER DYED 36 IN/90 CM LENGTH SUTURE WITH TAPER POINT 1/2 CIRCLE 36 MM-37 MM LENGTH DOUBLE-ARMED NEEDLE, 1 SUTURE PER PACK is now Optional Item 48. NOTE: Vendor must provide photographic images of required items 5.24, 5.83, and 5.146, in support of requirements #8, 9, 10, 11 & 12. For suture product box packaging vendor must submit a legible photograph of all sides of box packaging that contain product information. For individual product packaging, vendor must submit a legible photograph of front and back of product�s individual package. Graphic templates of product labeling are NOT ACCEPTABLE supporting documentation. Vendor must provide a letter of attestation, including a list of part numbers for the required items 5.1-5.23, 5.25-5.82, 5.84-5.145, 5.147-5.169, stating that the offered products meet requirements #8, 9, 10, 11 & 12. Vendors interested in participating in this standardization action need to submit updated QSL Package Worksheet with all of the supporting documentation by 01 April 21 5:00 PM CST. There are no other modifications at this time.
