Description
This is a Sources Sought Notice Only. This is not a request for quote; the VA is not seeking pricing or offers. The Department of Veterans Affairs, Network Contracting Office (NCO) 10, is currently seeking potential sources/vendors having the skills and capabilities necessary to provide three (3) Board Certified/Board Eligible Psychiatrists to provide outpatient/inpatient psychiatry services for the VA Northern Indiana Health Care System (VANIHCS) at the following locations: Marion VAMC, 1700 E. 38th St., Marion, IN; Ft. Wayne VAMC, 2121 Lake Ave., Ft. Wayne, IN; St. Joseph County VA Clinic, 1540 Trinity Place, Mishawaka, IN; Community Based Outpatient Clinic (CBOC) Muncie VA Clinic, 3401 S. Morrison Rd., Muncie, IN; and Community Based Outpatient Clinic (CBOC) Peru VA Clinic, 750 N. Broadway, Peru, IN. The general assignment will be five (5) days at either the Marion, Ft. Wayne VAMC and/or St. Joseph County VA Clinic. The assignment consists of weekend rounds at Marion VAMC and/or on call at the Ft. Wayne VAMC, includes Saturday clinic coverage as needed, and assistance at the Muncie and Peru CBOC, but may change depending on workload. Contract provider will generally work (8) hours per day (40) hours per week. This requirement is for a one (1) year contract with four (4) one (1) year option periods to be exercised at the discretion of the Contracting Officer. The anticipated period of performance: 7/1/2021 through 6/30/2022. The NAICS Code assigned to this is requirement is 621112, Offices of Physicians, Mental Health Specialists. All interested contractors are invited to respond, as this information may be used to determine potential set asides for the above noted services. No reimbursement will be made for any costs associated with providing information in response to this notice or any follow up information. If this is a service that your business can provide respond to this sources sought notice and include the following information: Company name and address. Capabilities/Qualifications: A written response providing clear and unambiguous evidence to substantiate the capacity to fulfill this requirement. Description of the capabilities/qualifications/skills your company possesses to perform services described. Do you need to subcontract any portion of work to provide these services stated herein? Past Experience relative to similar requirements. Offer may provide the following information on similar projects for which the responder was a prime contractor. The name, address, and value of each project. The Prime Contract Type, Firm Fixed-Price. The name, telephone and address of the owner of each project. A description of each project and why it is relevant to this requirement. Include difficulties and successes. Your company s role and services provided for each project.
