Page 3 of 3 The Department of Veterans Affairs Network Contracting Office (NCO)-16 in Ridgeland, Mississippi in support of the Veterans Health Care System of the Ozarks in Fayetteville, AR 72703 intends to issue and award a Sole (Single) Source award firm-fixed price type contract to Iron Bow Technologies, LLC., 2303 Dulles Station Blvd., Ste. 400., Herndon, Virginia, 20171-6353, for the purchase of clinical access stations for activation on proprietary manufacturer equipment at the Veterans Healthcare System of the Ozarks located at 1500 Dodson Ave, Fort Smith, Arkansas 72917. It has been determined that through initial Market Research, only one responsible source can perform the requirements via the attached Scope of Work (SOW) in accordance with Federal Acquisition Regulation (FAR) 6.302-1 Only one responsible source and no other supplies or services will satisfy agency requirements . No award will be made based on quotes or offers received in response to this notice. All responsible sources may identify their interest to meet this requirement to Jeneice McClinton, by 12:00p.m. (Central Standard Time), March 25, 2021. Additionally, any response to this notice must show clear, compelling and convincing evidence that competition will be advantageous to the government by providing the following: 1. Statement of Capability: Submit a brief [five (5) pages or less] capability statement explaining your capability to perform services for the Veterans Health Care System of the Ozarks. Include experience in performing these services for the VA, other Government (Federal or State) agency, or for a private medical facility. 2. Business Size and Socio-Economic Status: (a) Indicate whether your business is large or small (b) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business (c) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act (d) Indicate if your firm is a certified Hub zone firm (e) Indicate if your firm is a woman-owned or operated business (f) Indicate if your firm is a Vendor Information Pages (VIP) verified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) (g) Include the DUNS number of your firm. (h) State whether your firm is registered with the System for Award Management (SAM) at www.sam.gov and/or the VetBiz Registry at www.vetbiz.gov . If not, please NOTE: any future solicitation could only be awarded to a contractor who is registered in SAM. To receive credit as a SDVOSB or VOSB status you must be registered in the VetBiz Registry. 3. Authorization: 3.1 Are you the manufacturer? 3.2 Can you meet the non-manufacturer s rule? 3.3 Are you an authorized vendor, authorized distributor or authorized partner for providing medical equipment of clinical access stations and capable to meet all government requirements and objectives? (Prospective non-original equipment manufacturer (OEM) contractors MUST provide an authorization letter from manufacturer identifying they are an OEM authorized vendor, authorized distributor, authorized partner authorized to purchase clinical access stations, evidence of proof verified by an authorization letter on OEM letterhead or other document of valid purchase to obtain supplies and proof of access to software/updates from the OEM. All software licensing, warranty and service associated with the clinical access stations shall be in accordance with the OEM terms and conditions). 3.4. Do you have an existing Blanket Purchase Agreement (BPA), General Services Administration Schedules (GSA) or Veteran Affairs (VA) Federal Supply Schedule (FSS) contract? If yes, please include the contract details and a brief summary of the products and services provided in your response. 3.4. Is your company capable of meeting all software licensing, warranty and service associated with the equipment and/or services such that the system shall be in accordance with the OEM terms and conditions?
