Attachment 1: Combined Synopsis-Solicitation for Commercial Items Page 3 of 5 Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-05 dated 3/10/21. This solicitation is set-aside for SDVOSB The associated North American Industrial Classification System (NAICS) code for this procurement is 561720 (Janitorial Services), with a small business size standard of $19.5M. The FSC/PSC is S201 (Housekeeping Custodial Janitorial). The Zanesville Community Based Outpatient Clinic (CBOC), 2800 Maple Avenue - Zanesville, OH 43701, is seeking to acquire a non-personal services contract to provide janitorial services. The contractor shall provide all management, tools, equipment and labor necessary to ensure that custodial services are performed at the 9,700sf CBOC in a manner that will maintain a satisfactory facility condition and present a clean, neat and professional appearance, IAW the Performance Work Statement (PWS). There is a possibility these services will be absorbed into the leasing contract for this location starting 1 October 2023 or at some point within the last option period. If this occurs, the last option may not be exercised or the final option period may be terminated for convenience at some point during this time.
