VHAPM Part 813.106 SAP: Soliciting Competition & Evaluation of Quotations SOP Page 3 of 5 Original Date: 10/12/17 Revision 04 Date: 06/10/20 Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2021-03 (eff. 02/16/2021). This solicitation is set-aside for SDVOSB The associated North American Industrial Classification System (NAICS) code for this procurement is 335999, with a small business size standard of 500 Employees. The FSC/PSC is 6515. The NCO 22G Department of Veteran Affairs 4135 S. Powers Rd Mesa AZ 85212 is seeking to purchase Tri W-G Hi-Lo Mat Platform. All interested companies shall provide quotations for the following: THIS IS A BRAND NAME ONLY THE BRAND NAME IS TRI W-G. Supplies/Services Line Item Description Quantity Unit of Measure Unit Price Total Price 0001 TRI W-G HIGH HI-LO MAT PLATFORM, 6'X 8', PEARL GREY, REGIMENTAL BLUE UPHOLSTERY 3 EA 0002 TRANSPORTATION & INSTALLATION 1 EA Statement of work The Physical and Occupational Therapy request the purchase of three (3) Tri W-G High Hi-Lo Mat Platform tables. These are three (3) motorized hi-low mat tables for the provision of physical and occupational therapy treatment. These are to replace three (3) mat platform tables currently in use within the Service that are beyond economical repair and past end-of-life (purchased in 1987). These tables are used by therapy providers multiples times daily to provide occupational and physical therapy treatment and exercise instruction to Veteran patients. We have the following technical specifications for the Tri W-G Hi-Lo Mat Platform: Physical Dimension of 6 X 8 Construction: 10-gauge steel base and frame with rounded corners and PVC bumper guards to prevent scuffs, tears and damage from wheelchairs Minimum 600lb weight capacity 2 upholstered top surface Color desired: Pearl Gray steel base with Regimental Blue upholstery Four legs with four-corner stability Motorized height adjustment of approximately 23 to 35 Pneumatic Foot switch for hands free operation Delivery shall be provided no later than 60 days after receipt of order (ARO). Delivery terms will be FOB destination. Power Road Suite 103, Mesa, AZ, 85212, Jasmon Cornick, Contract Specialist, at jasmon.cornick@va.gov The following are the decision factors: Award shall be made to the quoter whose price is the lowest and meets all requested salient characteristics. The government will evaluate information based on the following evaluation criteria: meeting all listed salient characteristics and price. The award will be made to the response most advantageous to the Government. Responses should contain your best terms, conditions. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: ""The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."" Quoters shall list exception(s) and rationale for the exception(s), if any. Submission of your response shall be received not later than 14:00 PM EST, 3/24/2021 at jasmon.cornick@va.gov. Quotes will only be accepted via email. Hand delivered or mailed quotes will not be accepted. Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212-1(f). No late quotes will be accepted. Any questions or concerns regarding this solicitation should be forwarded in writing via e-mail to the Point of Contact listed below no later than 10:00 AM EST 3/22/2021. The FMS Vendor File Update form needs to be submitted in all cases to the Contracting Officer as part of the offer or proposal. Prior information sent to the VA Payment Center in Austin does not preclude submittal of the FMS Vendor File Update form. Failure to include this information could preclude the offeror from consideration for award or considerably delay award. Additionally, any/all Service Disabled Veteran Owned Businesses who respond to this solicitation must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov by the end date/time of this solicitation.
