- MARKET SURVEY REQUEST: THIS IS A REQUEST FOR INFORMATION ONLY for market research and planning purposes only, as defined in FAR 15.201(e). This is not a solicitation or request for competitive proposals. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. It is not to be construed as a commitment by the Government nor will the Government pay for the information solicited. No solicitation document exists or is guaranteed to be issued as a result of this Notice. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted, and the Government will not reimburse respondents for preparation of the response. Response to this Notice is strictly voluntary and will not affect future ability to submit an offer if a solicitation is released. There is no bid package or solicitation document associated with this announcement. BACKGROUND: The Office of the Armed Forces Medical Examiner Systems (AFMES) is the center of medical-legal investigations for the organization, and is responsible for determining the cause and manner of death for all active duty members who die within federal jurisdiction, as well as for identifying the decedent. The Office of the Armed Forces Medical Examiners (OAFME) also provides assistance with medical legal investigations, aerospace pathology & air mishap investigations, and medical mortality surveillance. REQUIREMENT: The purpose of this contract is to provide Maintenance Support Services on all LODOX Brand X-Ray Unit Equipment located at the Office of the Armed Forces Medical Examiners (OAFME). The Contractor shall provide preventative and/or corrective maintenance services to the OAFME. The Contractor shall be responsible for all-inclusive parts, labor, materials, travels, and trained personnel, supervision of trained personnel, telephone support, hardware/software updates/upgrades, and shipping/freight costs necessary to perform all maintenance actions covered under this contract. The equipment listed below shall be maintained within the Original Equipment Manufacturer (OEM) specifications, in like new or better condition: Manufacturer: LODOX Description: X-Ray Unit Maintenance will include, but not be limited to: regularly scheduled monthly, quarterly, and/or yearly preventative maintenance, inspections, calibrations, and corrective/repair maintenance tasks. MAINTENANCE SERVICE PERSONNEL QUALIFICATIONS: Personnel assigned to or utilized by the contractor in the performance of this contract shall be fully capable of performing maintenance on a LODOX Brand specific X-Ray Unit in an efficient, reliable, and professional manner. Contractor personnel must be fluent and easily understandable in English. All work shall be performed by service personnel with the proper Original Equipment Manufacturer (OEM) service training for the products which they will perform service. The Contractor shall ensure their personnel have appropriate hazardous materials training. SUBMITTALS: Summaries of the services as listed above shall be no more than Five (5) pages in length, and shall describe service capabilities in each of these areas.�� Interested Parties shall provide business size status based upon NAICS code 811219, Other Electronic and Precision Equipment Repair and Maintenance, a Small Business Size Standard of $22 Million, CAGE code as registered at SAM.gov. Interested Parties may include item brochure information with the summary package by the Government's requested date of submittal of 30 March 2021. Interested parties shall submit information in response to this RFI to: jennifer.s.bassett.civ@mail.mil.�� Responses shall be submitted and received via Email no later than 2:00PM EST Tuesday, 30 March 2021. Documents shall be electronic in Microsoft Word, Excel, Power Point, and/or Adobe Portable Document Format (PDF) and provided as attachments to the Email. All information received in response to this RFI that is marked proprietary will be handled accordingly. Responses to this RFI are not offers for a contract and will not be used by the Government to form a binding contract.
