Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation no. 75H71321Q00010 has been issued as a Request For Quotation for a firm fixed price supply contract for brand name or equal medical supplies. The performance period includes a one-year Base and 4 one-year options. The required delivery date for medical supplies. The delivery location and place of acceptance is Western Oregon Service Unit aka Chemawa Indian Health Center, 3750 Chemawa Road NE Salem, OR 97305, F.O.B. Destination. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-09 dated 08/27/2020.
