REQUEST FOR INFORMATION IS ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION PURPOSE This is a Request for Information (RFI), this is not a Request for Quotes (RFQ). No formal solicitation document exists at this present time. This is not a solicitation announcement. The purpose of this Sources Sought Notice is for market research purposes only to gain knowledge of potential SDVOSBs or VOSBs qualified under NAICS code 339112, PSC: 6515 that manufacture or are authorized to distribute the requested supplies on behalf of the manufacturer. Responses to this Sources Sought Notice will be utilized by the Government to make appropriate acquisition decisions. After review of the responses to this RFI, a solicitation announcement may or may not be published in Contract Opportunities. If it's found this requirement is considered ""Sole Source"" - there is high chance of an ""Intent to Sole Source"" notice being posted for the award to go to the one manufacturer/distributor. DESCRIPTION The current estimated usage provided from the end user is 4 per year of each Catheter listed below. S4IVL4040 IVL Catheter (Shockwave), 4.0x40 mm (estimated usage is 4 per year) M5IVL5060 IVL Catheter (Shockwave), 5.0x60 mm (estimated usage is 4 per year) M5IVL6060 IVL Catheter (Shockwave), 6.0x60 mm (estimated usage is 4 per year) M5IVL7060 IVL Catheter (Shockwave), 7.0x60 mm (estimated usage is 4 per year) REQUESTED INFORMATION If you are capable to deliver/perform the supplies/services, please provide the requested information as indicated below. Responses to this Sources Sought Notice shall also include: company name, address, point of contact, and size of business: - Does your company have an FSS contract with GSA or the NAC or are you a contract holder with NASA SEWP or any other federal contract? If so, please provide the contract type and number. - Please indicate the size status and representations of your business, such as but not limited to: Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) with an active registration in VETBIZ, HUBZone, Woman Owned Small Business (WOSB), Small Business, Large Business, etc.). NOTES All Capability Statements/Responses to this Sources Sought Notice shall be submitted via email only to David.Valenciabonilla@va.gov. Telephone, fax, or any other form of communication inquiries will not be reviewed or accepted. If a solicitation is issued, it may or may not be announced at a later date, and all interested parties shall respond to that solicitation announcement separately from the responses to this Sources Sought Notice. DISCLAIMER This RFI is issued for information and planning purposes only and does not constitute a solicitation. Responses to the RFI will not be returned. All information received in response to this RFI that is marked a proprietary will be handled accordingly in accordance with (IAW) Federal Acquisition Regulation (FAR) 15.201(e), RFIs may be used when the Government does not presently intend to award a contract, but wants to obtain price, delivery, other market information, or capabilities for planning purposes. Responses to these notices are not offers and cannot be accepted by the Government to form a binding contract.
