- Document Type: Pre-Solicitation Notice Solicitation Number: W50S8Y-21-R-5001 Classification Code Q518 � Medical Physical Medicine/Rehabilitation Set Aside: Total Small Business NAICS Code: 621399 � Offices of All Other Miscellaneous Health Practitioners The Oregon Air National Guard at Portland Air National Guard Base intends to issue a Request for Proposal (RFP) to award a single firm fixed- price contract for services, non-personal, to provide all personnel, equipment, tools, materials, supervision, and quality control necessary to perform Human Performance Program (HPP) services. The Contractor shall perform injury prevention and mitigation, rehabilitation services, and human performance optimization services to personnel assigned to the 125 Special Tactics Squadron (STS), 142 Fighter Wing (FW), Portland, OR, USA. The Contractor shall furnish consultation and reports, similar to the range of services found in a commercial facility, as well as services specialized to meet the needs of Air Force Special Warfare operators. Performance shall be in accordance with the professional standards of the Accreditation Association for Ambulatory Health Care (AAAHC), and as defined in the Performance Work Statement. The North American Industry Classification System (NAICS) code for this work is 621399. The small business size standard is $8 million. This action is being procured on a total small business set-aside basis. The tentative date for issuing the solicitation is on-or about 20 November 2020. Due to COVID-19 precautions and the nature of the services required a site visit will not be conducted. All questions for the must be submitted by 11 December 2020 via email to andrea.smothers@us.af.mil. The solicitation closing date is scheduled for on-or about 22 December 2020. Actual dates and times will be identified in the solicitation. The source selection process will be conducted in accordance with FAR Part 15.3 and DFARS Part 215.3, source selection procedures. Evaluation factors include Technical, Past Performance and Price. The Government intends to award without discussions. Interested offerors must be registered in the System for Award Management (SAM).
