Statement of Work Medical Supply Prime Vendor Liaison Louis A. Johnson Medical Center Clarksburg, WV Logistics Representative: AcquisitionUtilizationSpecialistTeamClarksburg@va.gov Contracting Officer s Representative: AcquisitionUtilizationSpecialistTeamClarksburg@va.gov Background The Louis A. Johnson Medical Center, Clarksburg, WV Veterans Service Network (VISN) 5 needs two Medical Supply Prime Vendor Liaisons (MSPVL). These two positions will be assisting the facility in purchasing equipment and supplies through the Medical Supply Prime Vendor (MSPV). Summary The Medical Supply Prime Vendor Liaison shall aid participating facilities in identifying opportunities to use the Medical Supply Prime Vendor (MSPV) contract fully and efficiently. The Medical Supply Prime Vendor Liaison shall be fully knowledgeable of the medical/surgical supplies and services offered via the MSPV contract, and medical/surgical supplies and services in general. Position Description The Medical Supply Prime Vendor Liaison is a full time on site/virtual representative and expected (at a minimum) to perform the following functions: Assist the participating Facility in the identification of opportunities to utilize the mandatory MSVP contract, in achieving the contractual fill-rate fully and efficiently. Coordinate and introduce new master list products to the individual facilities. Review daily, weekly, and monthly reports that identify to the Facility COR or Supply Chain Management Designee product shortages or backorders and assists with the resolution thereof. Assist Facility staff in maintaining up to date MSVP pricing and federal contract information. Researches and coordinates approved back order substitutes. Shall hold conference calls, if/when requested, between VA representatives and the PV(s) to encourage open communications and/or to resolve contract administration issues. The MSPVL shall be fully knowledgeable of the medical/surgical supplies and services offered via the MSVP contract, and medical/surgical supplies and services in general, when they are provided as a value-added service. The Medical Supply Prime Vendor Liaison will use Government Furnished Equipment (GFE) in the performance of his or her duties. The Medical Supply Prime Vendor Liaison may be required to apply for a Government issued PIV Card, which must be returned on the date of contract expiration. The on-site services provided shall not be personal services as defined by FAR 2.101, and the contractor retains all supervisory responsibilities of their on-site representatives. Qualifications The Medical Supply Prime Vendor Liaison must meet the minimum qualifications of at least one year of Business Analytics experience in the private medical commodity industry or within the Federal Government; Bachelor s degree preferred. Strong analytics and skill with ADP systems, along with strong communication skills both written and verbal is a must. Must be able to communicate with and direct representatives/managers to action through reports constructed as part of their analysis. Must have strong problem-solving skills. Knowledge of SAP is a plus. Changes in these qualifications require VA/VHA Facility management approval. Work Schedule/Place of Performance The Medical Supply Prime Vendor Liaison is required to report for duty Monday through Friday, during the facility s core working hours unless the COR and the MSPV agree on an alternate schedule. Contractor policy regarding excused absence from duty due to authorized/approved holidays, sick days and vacation days shall apply to the MSPVL. Exact working schedule will be determined by the contractor and the COR. Overtime may be available to meet facility needs. Wage rate determination will follow all applicable laws. Federal Government Holidays are indicated below, except for holidays designated by Presidential declaration. New Year s Day, January 1 Martin Luther King s Birthday, 3rd Monday in January President s Day, 3rd Monday in February Memorial Day, Last Monday in May Independence Day, July 4 Labor Day, 1st Monday in September Columbus Day, 2nd Monday in October Veterans Day, November 11 Thanksgiving Day, 4th Thursday in November Christmas Day, December 25 Influenza Vaccination VHA Directive 1192 requires all health care personnel (HCP) to participate in the seasonal influenza prevention program and outlines the key implementation steps. Health care personnel is defined in VHA Directive 1192 as all VA licensed and unlicensed, clinical and administrative, paid and unpaid, full- and part-time employees, intermittent employees, fee basis employees, VA contractors, students, researchers, volunteers, and trainees who work in VHA locations. HCP covered by the policy are expected to receive annual influenza vaccination. This requirement is extended to all Contractor personnel with potential to come into minimal contact (passing in the corridor) with any patients, visitors, or staff members at VAPHS. Those individuals unable or unwilling to be vaccinated are required to wear a face mask throughout the influenza season. Masks are available at entrances throughout the medical center. It is the responsibility of the contractor to ensure that all contract staff is compliant with the requirements outlined in VHA Directive 1192. The contractor shall maintain the following documentation: - Documentation of vaccination, e.g., signed record of immunization from a health care provider or pharmacy, or a copy of medical records documenting the vaccination. - Completed Health Care Personnel Influenza Vaccination Form (Attachment B of VHA Directive 1192) if unwilling or unable to receive the vaccine. The contractor is required to submit annual certification in the form of a memorandum to the Contracting Officer s Representative (COR) that all contract staff performing services at VA facilities is in compliance with VHA Directive 1192. Tuberculosis Prevention Standard Personnel Testing (PPD, etc.): Contractor shall provide proof of the following tests for personnel within five (5) calendar days after contract award and prior to the first duty shift to the COR and Contracting Officer.  Tests shall be current within the past year. TUBERCULOSIS TESTING:  Contractor shall provide proof of a negative reaction to PPD testing for all contract personnel.  A negative chest radiographic report for active tuberculosis shall be provided in cases of positive PPD results.  The PPD test shall be repeated annually. RUBELLA TESTING:  Contractor shall provide proof of immunization for all contract personnel for measles, mumps, rubella or a rubella titer of 1.8 or greater.  If the titer is less than 1.8, a rubella immunization shall be administered with follow-up documentation to the COR. Please provide a signed memo stating that: (number of assigned Key Personnel) assigned to VA Contract #___________ have tested negative for tuberculosis within the past 365 days. (number of assigned Key Personnel) assigned to VA Contract #___________ have been immunized for measles, mumps, rubella or a rubella titer of 1.8 or greater.
