- The Veterans Health Care System of the Ozarks (VHSO) Dental Service in Fayetteville, AR and Springfield, MO are requiring services for fabrication and repair of Dental Fixed and Removable Prostheses. The Contractor shall provide all resources necessary to fabricate and repair Dental Fixed and Removable Prostheses for dental patients eligible for treatment at the VHSO Dental Service, delivering in a manner specified by the Schedule for Deliverables. General Requirements: Primary Laboratory Services Contractor shall provide the following dental laboratory services to the VHSO Dental Service. The Contractor shall provide all materials and supplies necessary to complete each item. For every task listed in the solicitation, the Contractor shall identify in writing all necessary subtasks (if any) and associated costs. Service(s) that is/are not listed in this solicitation must be added as a fixed price Service Line Item Number (SLIN). All contract modifications must be completed by the VA Contracting Officer. Fabrication of Valplast Partials Repairs of Valplast Partials Fabrication of Cast Metal Partials Repair of Cast Metal Partials Fabricate Crowns for Implant Abutments Fabrication of Gold and Porcelain Crowns Fabrication of Removable Complete Dentures and Partial Dentures Augmented Laboratory Services VHSO Dental Service may request augmented services. The Contractor shall provide all materials and supplies necessary to complete each item. Contractor shall provide the following dental laboratory services to the VHSO Dental Service. For every task listed in the solicitation, the Contractor shall identify in writing all necessary subtasks (if any) and associated costs. Service(s) that is/are not listed in this solicitation must be added as a fixed price Service Line Item Number (SLIN). All contract modifications must be completed by the VA Contracting Officer. Fixed Prosthetic Crowns (precious, non-precious, Porcelain fused to Metal (PFM) Ceramic) Traditional acrylic partials with wire clasps Crowns for implants Splints Mouth Guards Processed Hard and Soft Relines or Rebases Porcelain fused to metal crowns and bridges All gold crowns and bridges All ceramic crowns and bridges Processed provisional crowns and bridges Diagnostic wax-up and design services for removable and fixed prosthodontics Pre-fabricated and custom abutments for implants Implant supported crowns and bridges Maryland bridges Precision milled retentive bars Precision and semi-precision attachments Cast gold post and cores Partial denture frameworks Implant supported complete and partial dentures Relines and Repairs The Contractor shall provide all materials and supplies necessary to complete each item. Acceptable supplies for removable prosthodontics include: Lucitone 199 denture acrylic and Dentsply Portrait denture teeth. Acceptable supplies for fixed prosthodontics include: type 3 gold, noble metals and quality porcelain systems such as IPS Empress, Procera, and Zirconia. REQUIRED CRITERIA The Contractor shall provide items that meet the quality, esthetic and functional requirements of the patient. All dentures and partials will carry a patient identification. The Contractor shall be familiar with mould guides by Dentsply Portrait and with shade guides by Dentsply Portrait. Materials utilized shall be from ISO 9001/CE registered companies. The Contractor shall use procedures, techniques, and current technology, which are considered to meet Federal Drug Administration (FDA) regulations and American National Standards Institute (ANSI) guidelines. The Contractor shall not subcontract any of VHSO dental laboratory services for the duration of the contract. The following materials shall be used in the fabrication of dental prostheses: Complete Dentures/Removable Partial Dentures Lucitone 199 acrylic resin Color coloration or equivalent Occlusal Scheme: Monoplane, linear Anatomic, full balanced Lingualized, linear 33 */monoplane 30 *rational Functional/rational Tooth material: Dentsply Portrait The Contractor shall at no additional expense to VHSO, re-make all dental items that are not correct, cannot be properly seated, or flawed due to Contractor error. If, in the case of porcelain work, the porcelain shatters within twelve months of fabrication, it shall be considered flawed and remade at no additional expense to VHSO. The Contractor's laboratory shall provide: Case consultation with individual VHSO Dentists Responsive and knowledgeable customer service representatives at no cost Patient identification placed on all dentures and partials that consists of initial of last name and last four numbers of social security number Fabrication and repairs conducted in-house/on-site No charge for daily pick-up, tracking and delivery services Oversight and administration of pick-up and delivery services Establish and maintain a record keeping system that will track deliveries to VHSO Dental Service and the return of services requested from the Contractor's location of service Detailed Personalized Prescription Pads Shipping containers, packing materials and shipping labels Adherence to The Centers for Disease Control (CDC) infection control guidelines for all incoming and outgoing cases. A Quality Assurance Program: Quality Assurance Process (QA) that will guarantee a product that meets the patient's natural esthetics & superior fit with minimal chair time. Use of materials purchased from ISO 9001/CE registered companies. No lead products or by-products will be used in any fabrications or repairs. Contractor shall perform internal technical audits to ensure consistent, high quality, and timely restorations. The Contractor's laboratory shall use procedures, techniques, and current technologies, which are consistent with Occupational Safety and Health Administration (OSHA) standards. All prostheses will be fabricated in the United States of the America using materials approved by the Food and Drug Administration (FDA).
