- FROM: 763 Enterprise Sourcing Squadron 510 POW/MIA Suite E1001 Scott AFB, IL 62225 � 5022 SUBJECT: Simplified Acquisition Quote Request (SAQR), FA4452-21-Q-0003, Medical Equipment Rail Kit (MERK) 1. This is a Simplified Acquisition Quote Request (SAQR), for subject to be awarded pursuant to Federal Acquisition Regulation (FAR) Part 13 and the specific requirement outlined in this request with attachments. Headquarters (HQ) Air Mobility Command (AMC)/Surgeon General Requires a combined MERK and hard-sided case manufactured by SMEED Technologies that will support both the operational use and contingency storage of the MERK while in War Reserve Material (WAR) Storage. 2. It is the intent of the 763 Enterprise Sourcing Squadron (763 ESS) to award a Single Source, firm-fixed priced (FPP) contract to SMEED Technologies located at 7450 at Gentry Way, Cumming, GA 30041, for the subject requirement. 3. There are five attachments to this SAQR: Attachment 1- Statement of Work (SOW), Attachment 2- Information Sheet, Attachment -3 Provisions and Clauses Attachment 4 - Section 889 Representation, and Attachment 5 � Single Source Justification and Authorization (J&A). A response to this request shall consist of completion of Attachment 2 and applicable specifications (or a submitted quote in contractor format as long as it, at minimum, depicts unit and extended prices (rounded to the nearest dollar), applicable specification and delivery periods) and attachment 3 Provisions and Clauses, and Attachment 4-Section 889 Representation.. 4. The Government intends to utilize North American Industry Classification System (NAICS) code 541611, with corresponding business size standard of $16.5 million and Product Service Code (PSC) R799. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition FAC 2021-04. Offerors must be registered in System for Award Management (SAM) to be considered for award. Offerors may register at www. Sam.gov. Duns and Bradstreet (DUNS) number is required in order to register. 5. Basis of Award: Award will made IAW FAR Part 13 procedures and the procedures specified in this SAQR. The Government will make a determination of contractor responsibility IAW FAR Subpart 9.1, as supplemented, prior to issuing an award. Prices will be determine fair and reasonable through the use of one or more of the evaluation techniques at FAR 13.106- 3(a)(2). 6. The Government intends to make an awarded based on the initial proposal submission without conducting interchanges. Therefore, the offer should contain the Offeror�s best terms from a price and technical standpoint. However, the Government reserves the right to hold interchanges if, during the evaluation, it is determined to be in the best interest of the Government. Interchanges are fluid interaction(s) between the Contracting Officer (CO) and the contractor that may address any respect of the proposal and may or may not be documented in real time. However, the Government reserves the right to conduct interchanges using Interchanges Notices (IN). Offeror responses to IN will be considered in making the award selection decision. Interchanges may be conducted with one, some or all offers as the Government is not required to conduct interchanges with any or all contractors responding to this SAQR. 7. This SAQR is not authorized to begin performance, and is no way obligates the Government for any costs incurred by the offeror associated with developing a proposal. The Government reserves the right not award a contract in response to this SAQR. Prior to commencement of any activities associated with performance of this requirement, the Government will issue a written directive or contractual document signed by the Contracting Officer with appropriate consideration established 8. Quotes are due by 12:00 PM (Central Standard Time on Friday, 9 April 2021). Submit quotes by email to Mr. Joseph B.
