Description
The Regional Health Contracting Office - Atlantic, Gordon Health Contracting Branch Fort Gordon, Georgia, has a requirement for equipment and transportation necessary to supply a high energy electrohydraulic extracorporeal shockwave therapy device to include any components for use in chronic plantar fasciitis. In addition, the this requirement will require fully insured and trained operator on this equipment who will not only provide intra-operative operation of the shockwave therapy machine but will perform installation, set-up, and preventative maintenance verification at Eisenhower Army Medical Center, Fort Gordon, Georgia; Blanchfield Army Community Hospital, Fort Campbell, Kentucky; Martin Army Community Hospital, Fort Benning, Georgia; and Winn Army Community Hospital, Fort Stewart, Georgia. This will be an all or nothing for a Period of Performance-15 May 2021 through 14 May 2026. �THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY. This notice does not constitute a commitment by the Government. Interested parties are asked to submit capability statement that clearly and convincingly identifies their capability to fulfill the requirements.� SALIENT CHARACTERISTICS: FDA approved Class III medical device, and the only FDA approved high-energy ESWT device to treat both chronic plantar fasciitis and chronic lateral epicondylitis. Additional characteristics include: - Variable cycle speed, 1-8 cycle per minute - Exact measurable movement - On-the-fly changes to angle, speed and hold time - Sliding face and lower sections for comfort during motion - Movement: adjust flexion, adjust extension or both in one cycle - Angle: flexion and extension controlled independently by doctor Detailed capabilities must be submitted by email to Contract Specialist,� Melissa Teer at melissa.d.teer.civ@mail.mil and Contracting Officer, Sebrena Lane at sebrena.l.hackleylane.civ@mail.mil in an electronic format, no later than 12 pm Eastern, 1 April 2021. DO NOT SUBMIT CLASSIFIED MATERIAL.As part of market research, this Sources Sought is to determine if there exist an adequate number of qualified interested contractors capable of providing the requirement. The Government may use the responses to this Sources Sought for information and planning purposes.� The applicable NAICS code for this requirement is NAICS 532490 Other Commercial and Industrial Machinery and Equipment Rental and Leasing with a Small Business Size Standard of $7.5 MIL. The Product Service Code is 6515-Medical and Surgical Instruments, Equipment and Supplies. Businesses of all sizes are encouraged to respond; however, each respondent must clearly identify their business size in their capabilities statement. This Sources Sought is only for the purpose of identifying potential sources as part of market research. No Request for Proposal (RFP), Quote (RFQ), or Invitation for Bid (IFB) exists. Responses to this Sources Sought are not quotes, proposals, or bids on which Gordon Health Contracting Branch can issue any contract. This Sources Sought is issued for information and planning purposes only and does not itself constitute an RFP, RFQ or IFB. The Government does not intend to award a contract based only on responses to this Sources Sought. Responses to the Sources Sought will not be returned. Responders are solely responsible for all expenses associated with responding to this Sources Sought.� The Fort Gordon Contracting Branch will not pay for information received in response to this Sources Sought. Do not submit pricing information in response to this Sources Sought.� If your organization has the potential capacity to provide the required services, please provide the following information: 1) Organization name, address, primary points of contact (POCs) and their email address, FAX, Web site address, telephone number, and type of ownership for the organization; and 2) Tailored capability statements addressing the requirements of this notice, with appropriate documentation supporting claims of organizational and staff capability.
