Statement of Need Background: The Lebanon VA Medical Center (LVAMC) has a requirement for one (1) Brand Name Only Olympus Thulium Laser, and accessories. Superior Stone Fragmentation, resulting in a number of advantages for the patient: Less likelihood of the need for a second procedure (either as planned for a large stone, or unplanned because of adverse intra-op findings). Shorter OR time, by ½ due to: The smaller 150 micron fiber allows for better flow of irrigant to clear the field of view Less bleeding, as inadvertent but inherent mucosal lasering is immediately coagulated, which does not occur with holmium energy Less sone debris because of superior fragmentation Less fragments that need to be passed Advantages for selected patients: Large renal or bladder stones that predictably would require more than one procedure Anticoagulated patients that bleed easily, which obscures visibility and prompts a second procedure. Large renal stones, that are currently being referred out for community care, could be treated here with the Thulium Laser Large bladder stones that are removed by incision would be treated endoscopically with the Thulium Laser. Overall, the Thulium Laser is a safer, and efficient way of dissolving calculi, and results in less surgical failure, as well as a cost savings to the facility as mentioned above. Additional Information: The Olympus Thulium Laser is the chosen model based on the proven effectiveness, all-in-one platform, small footprint, many of the features offered above not available from competitor models and dependability of the Olympus Product. Delivery: Delivery shall be within thirty (30) days from the time of award. The Thulium Laser and accessories shall be delivered to the Lebanon VAMC located at 1700 S. Lincoln Ave., Lebanon, PA 17042; Attn; Warehouse. BioMedical Engineering will need to perform a safety check and enter the equipment into the system for annual Preventive Maintenance (PM) checks.
