The RFI answers and Statement of Work can be found on the following pages. Proposals remain due NLT 2:00PM April 8, 2021 Lisa Lake Contracting Officer Page 4 of 4 Request for Information with answers 1.  What ICRA level is required on this project? Dust Control , elevator needs to be locked in the open Position , needs to have sticky pads at the foot of the elevators , needs to have fans when installing epoxy floor . 2.  What is the process for getting our PIV reactivated? PIV activation and/or reactivation will be discussed with the contractor that is awarded the contract 3.  What is the process for change orders in the event that the substrate requires unforeseen repairs?  Please see FAR 52.243-5 and VAAR 852.243-70 for procedures involving change orders and/or changed conditions STATEMENT OF WORK   1.  BACKGROUND:  This requirement is for Installation of 11 elevator floors and sub-floor for the Lyons Veterans Medical Facility, building on which is 306 acres, built in the early 1920 s with over 30 major buildings and a few smaller outbuildings.   2.  OBJECTIVES:  To provide better vigil care for patients and Employees that are in buildings throughout first circle and Bldg. 143. 3.  PLACE OF PERFORMANCE:  This project will be required for the Lyons campus: Lyons 151 Knollcroft Road Lyons, NJ 07939 4.  PERIOD OF PERFORMANCE:  45 days after award. 5.  SCOPE:  It is the intent of this contract to have 6 elevator 88 x59 and 5 elevators 96 x60 be installed with epoxy floor. All measurements are approximate. The service elevators in Bldg. 143 and Bldg9 Elevators need to have the sub-floor replaced before the epoxy floor is replaced.    6.  REQUIREMENTS:  1.The contractor shall provide all supervision, management, tools, supplies, materials, equipment, transportation, and labor in accomplishing the work under this contract.    2.The contractor shall follow all the Veterans Administrations, State, Federal, OSHA rules and safety regulations. 3. All workers will be experienced, and safety qualified with a valid 30-hour safety card.     7.  WORK TO BE PERFORMED:  Under this contract, the contractor shall complete the following tasks and requirements:  Contractor shall remove all floor tiles on existing floor on the list of elevators then dispose of all garbage and remove off site Contractor will install new sub-floors on the 2 service elevators in Bldg. 143 and Bldg9 using ¾ water resistant Plywood. 3.Contractor will be responsible for the removal and disposal of all Floor tile and Plywood 4.Contractor will mobilize at the site and provide containment where necessary. 5. Contractor will install a 100% solids epoxy screed to fill in any uneven areas. 6.Contractor will install a 100% solids Epoxy Decorative Flake System to the prepped floor area. 7. The application area will need to be roped off from traffic for approximately 36 hrs.   9.  GOVERNMENT-FURNISHED PROPERTY, MATERIAL, EQUIPMENT, OR INFORMATION:  Veterans Administration COR or Facility Manager will designate a Contractor Staging Area. No government property or equipment will be furnished to the contractor.  10.  SECURITY:  The Contractor can plan to work on site from 7 AM to 4:30 PM Monday thru Friday. Contractor cannot work outside of these hours or during federal holidays without prior approval from Facility Manager.   11.  OTHER UNIQUE REQUIREMENTS:   All work shall be performed by competent personal, experienced and qualified to work on the specific equipment. Contractor shall provide certification as required by the Joint Commission on the Accreditation of Healthcare Organizations (JCAHO) 12.  QUALITY ASSURANCE REQUIREMENT:  The contractor shall develop and maintain a quality control program to ensure services are performed in accordance with commonly accepted commercial practices.  The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. As a minimum the contractor shall develop quality control procedures addressing the critical areas identified in this statement of work. The government will periodically evaluate the contractor s performance in accordance with the statement of work. In addition, prior to the Contractor submitting a quote for the project a site visit shall be required. The Contractor shall be responsible for verifying all measurements prior to ordering materials.
