Attachment 1: Combined Synopsis-Solicitation for Commercial Items VHAPM Part 813.106 SAP: Soliciting Competition & Evaluation of Quotations SOP Original Date: 10/12/17 Revision 04 Date: 06/10/20 Description (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. (ii) This solicitation is issued as a Request for Quotations (hereafter RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-05, effective date 03-10-2021. (iii) This solicitation is Total Small Business Set-aside in the open market. (iv) The associated North American Industrial Classification System (NAICS) code for this procurement is 334516, with a small business size standard of 1000 employees. The FSC/PSC is 6630. (v) The Department of Veterans Affairs, VHA Regional Procurement Office West, Network Contracting Office 22 in Long Beach California is seeking to purchase quality control reagents & supplies for the Bio-Rad BioPlex 2200 Multiplex system and D-100 hemoglobin testing system. (vi) All interested companies shall provide a quote for all items listed on Attachment A, otherwise it will be considered non-responsive. See Attachment A for more details. 1) On Attachment A. tab 1. BioPlex, please provide extended price of all line items. 2) On Attachment A. tab 2. D-100, please provide extended price for 54, 000 estimated annual volume. SAVAHCS will provide monthly test volume by the 5th working day of each month based on LIS printout of patient volumes, QC runs and proficiency testing. (vii) The anticipated contract period of performance is 04/16/2021 to 04/15/2022. (xi) The Government will issue an order number and obligation number to the responsible quoter whose quotation conforming to the solicitation will be most advantageous to the Government, price and other decision factors considered. Award will be based upon a comparative evaluation of quotes in accordance with the Simplified Acquisition Procedures of FAR 13. Price reasonableness will be based on competitive quotations or offers. The Government is not requesting or accepting alternate quotations. Only one quotation per vendor will be accepted. (xii) The following are the decision factors: 1. Price: The Government will determine price reasonableness based on competitive quotations/offers received. Quotations/offers will be evaluated side-by-side comparison of vendors proposed prices. 2. Past Performance. * Note: The award will be made to the response most advantageous to the Government. The Government reserves the right to consider a quotation other than the lowest price that provides additional benefit(s). Responses should contain your best terms, conditions. (xiii) To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: ""The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."" OR ""The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:"" Quoters shall list exception(s) and rationale for the exception(s), if any. (xiv) All offeror quotes for this solicitation must be received electronically through email no later than Monday, April 12, 2021, 4:30 p.m. PST. Ensure to reference solicitation number 36C26221Q0560 in subject line of the email. Any offeror quotes received after the prescribed date and time shall be considered non-responsive and will not be considered for award. Any questions regarding this solicitation must be received electronically through email no later than Wednesday April 8, 2021, 4:30 p.m. PST.
