- THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY This is not a solicitation for proposals, proposal abstracts, bids, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Responses to the information requested will assist the Government in determining the appropriate acquisition method. The Great Lakes Acquisition Center (GLAC) - Southern Tier in Milwaukee, WI is soliciting businesses that can provide all necessary labor, personnel, supplies, travel, supervision, equipment, tools, and materials to provide Code Blue Short Range Paging System maintenance at the Edward Hines Jr. VA Hospital at 5000 South 5th Avenue, Hines, IL 60141-1457. This shall include a base year period of 05/01/2021-04/30/2022, with an additional Four (4) One (1) Year Options to be exercised at the discretion of the government. The NAICS code for the solicitation to be released is 517312, with a size standard of 1,500 employees. Service Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) Contractor(s) shall respond with their capabilities and proof of registration in VIP at https://www.vip.vetbiz.va.gov/Home/Index . Contractors shall also provide their DUNS number, business size and socioeconomic status under the NAICS Code listed above. All offerors must be registered in the System for Award Management (SAM) database at www.sam.gov prior to submission of any offer. Responses shall be due by 12PM CST on Monday April 12, 2021. Responses should be sent by email to richard.ruffin@va.gov. Requirement information is as follows: STATEMENT OF WORK 1. TITLE OF PROJECT Maintenance Contract for Hines Radio Paging System 2. SCOPE OF WORK: The contractor shall provide all resources necessary to accomplish the deliverables described in the statement of work (SOW), except as may otherwise be specified. Provide service and support for the Edward Hines Jr. VA Hospital s Code Blue Paging System. This system is already in place, and the vendor must keep this system in working order. 3. PERIOD OF PERFORMANCE: The period of performance is May 1, 2021 thru April 30, 2022 and 4 option years. Work at the government site shall not take place on Federal holidays or weekends unless directed by the Contracting Officer (CO). 4. TYPE OF CONTRACT: Firm-Fixed-Price 5. PLACE OF PERFORMANCE: VA Medical Center 5000 S. Fifth Ave, Hines, IL 60141 6. DESCRIPTION OF TASKS: a. The contractor shall provide service and support described below within the performance period. b. The Contractor shall provide the highest level of reliability with immediate, uninterrupted service 24 hours a day, 7 days a week. The VA operates medical centers which provide direct patient care and cannot lose telecommunication services. c. The current Code Blue Paging System has redundancies built into it, and these redundancies must be maintained. d. Contractor will ensure that the Code Blue Paging System in place at the Edward Hines Jr. VA Hospital continues to operate correctly 99% of the time. e. Vendor will respond within 4 hours of receiving a phone call from the COR or CO. f. VA employee will work with vendor over the phone in an attempt to rectify the situation. g. If the problem can t be fixed virtually then vendor will visit the facility within 72 hours at no expense to the Government. 7. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. 8. SECURITY The C&A requirements do not apply, and a Security Accreditation Package is not required
