Peters VA Medical Center 130 West Kingsbridge Road Bronx NY 10468-3904 POC: Lateisha Robinson | Lateisha.robinson@va.gov DESCRIPTION: NOTICE OF INTENT TO AWARD A SOLE SOURCE CONTRACT TO Veterans Healthcare Supply Solutions a certified distributor for Lombart Brothers for Lombart Reliance Optometry Chairs for the Northport VA campus. Market research was conducted and determined that Veterans Healthcare Supply Solutions is the only vendor capable of providing supplies necessary that meet the requirements. Optometry Exam Chairs Northport VA Medical Center, Northport NY Optometry Service (123D) Background: New exam chairs are required in Optometry service due to the EOF (end-of-life) of the existing chairs. The chairs were purchased in 1996 and are in unacceptable condition, especially regarding infection-control, with ripped vinyl coverings and exposed poly-fill material that can t be properly sanitized. They have been repaired many times by Biomed for coming off track, which is a safety hazard to both providers and patients. Headrests are no longer adjustable which limits head stability during procedures. The model requested meets all Service requirements and fits the physical space. The equipment has a long history of use within the clinic, staff is familiar with its functionality, and its proven durability adds extra assurance that it will remain in service for many years to come. Scope of Work: The Vendor shall deliver Ten (10) Reliance 7000-LFC chairs(#20 Pearl), ten (10) designer cushion sets (#301 Black), ten (10) Reliance 7900-IC Floor stands with instrument console (#20 Pearl), ten (10) 924 chair glides, and ten (10) Reliance Third Arm for the stands (#20 Pearl). The space for each chair is already accounted for within the clinic space and takes up the same area as the current chairs. Equipment Specifications: Memory Positioning Switch to enable pre-selection to the chair's most used position Power base and top Finger switches and corded foot switch to raise, lower, or recline the chair Square base Dual controls for base and top functions Membrane switches and rotation locks to control all operations of the chair from a sitting or standing position Single lock adjustable concave headrest standard Safety Switch to prevent small children from operating the chair by disabling side and footswitches Upholstered armrests that lock in place and release on both chair sides for security and comfort Release mechanism to allow patients to sit or leave from either side of the chair Padded underneath for comfort Hydraulic base tested to 600 pounds Large hinged footrest to support patient s full weight; tilts out of the way to save space Chairs will be placed on glides allowing access for any wheelchair bound patients. The glide system follows mandatory requirements outlined by the Americans with Disabilities Act. According to the vendor, the entire cost of the model requested may be applied toward tax credit provided by this legislation. Requirements: The vendor shall issue a credit for trade-in of the following equipment: Four (4) Reliance 7000 exam chairs each with a Reliance 7800IC exam stand. Six (6) Reliance 980 chairs linked to desks The Vendor shall remove poles and arms from six (6) desks. The Vendor shall attach slit lamps, phoropters and keratometers to new stands. Place of Performance: Northport VA Medical Center Optometry Service, Clinic 2J 79 Middleville Road Northport, NY 11768 Warranty: Products shall be warranted to be free from failures due to defects in materials or workmanship, for a period of one year (12 months) to include parts and labor. During the warranty period, the Contractor shall repair or replace without charge, any parts which are deemed to be defective. Contractor shall use OEM parts only. Patient Information Privacy and Protection: In accordance with Appendix A, Handbook 6500.6 Checklist blocks 6, the C&A requirements do not apply to this requirement, and a Security Accreditation Package is not required. There is no information to be protected. The equipment does not store any PHI/PII/PI, nor are there any hard drives/storage devices to service or remove.
