DeBakey VA Medical Center (MEDVAMC) is requesting a diagnostic analysis and repair of Philips EPIQ 7C Ultrasound Equipment. �Place of Performance: Michael E. DeBakey VA Medical Center, 2002 Holcombe Blvd, Houston, TX. 77030.� Item # Description/Part Number/Service* Qty Price Extended Amount �0001 Salient Characteristics for EPIC 7C Acquisition Module/Repair (1), 0002-Replacement Parts not included in Module �(1),:� 0003-Travel Time, (1), 0004-Labor for Ultrasound Diagnostic/Repair, (6), Suggested Manufacturer Brand Name; Philips: ITEM SPECIFICATIONS FEATURES: See Scope of Work for complete details, one year warranty on parts and labor.� Contractor shall hold and save the Government, its officers and agents, free and harmless from liability of any nature occasioned by the Contractor�s performance. �REQUESTED RESPONSES: The intent of this Request for Quote is to establish pricing restricted to certified sources. Interested contractors are requested to respond in accordance with the following: Please respond to this RFQ if you can provide the exact brand name products listed in the table. In the response please cite your business size status. If you have an existing GSA or VA, Federal Supply Schedule contract, please include the contract details in your response. DISTRIBUTORS: You must provide proof that you are an authorized distributor. VA does not accept grey market items. Please respond to this RFQ only if you can provide supplies that may be OEM products listed in the table in the background. Please provide details on the proposed products such as Manufacturer Name, Part Number, and Description. Vendors are requested to submit pricing with their responses. The pricing will be considered when determining fair and reasonable pricing and the best value to the government. Please note that if no responses to this notice are received, from either authorized distributors of the cited brand name, then this action will be sole sourced to the manufacturer cited in the Background section above.
