All other terms and conditions remain unchanged. Page 4 of 4 2. Scope of Work. VA Long Beach Healthcare System is seeking a contractor to perform PM and repairs on two dock lifts. 2.A. Specific Requirements. 2.A.1. The Contractor shall repair two (2) dock-lifts to include the following: 2.A.1.a. Reseal four (4) lift cylinders 2.A.1.b. Replace four (4) hydraulic hoses Power units to cylinders 2.A.1.c. Remove and reinstall lift cylinders 2.A.1.d. The following shall be new and provided by the contractor 2.A.1.d.i. Hyde hose approximately 200 feet 2.A.1.d.ii. Hyde fittings (8) 2.A.1.d.iii. Filter assembly (4) 2.A.1.d.iv. Hyde fluid approximately 20 gallons 2.A.1.d.v. Low pressure hoses approximately 10 feet 2.A.1.d.vi. Pin mounting (4) 2.A.1.e. Replace low pressure hose 2.A.1.f. Sump service (2) 2.A.1.g. Drain and replace hydraulic fluid 2.A.1.h. Dispose of freight and hazardous waste 2.A.1.h.i. Collect, contain and dispose of all debris daily and upon completion of work resulting from the overall scope of the project according to VHA Regulations. 2.A.1.h.ii. Contractor is responsible for repairing damages incurred during performance of this scope of work to include removal/installation or moving materials and tools in and out of the work area. 2.A.2. The Contractor shall work on one (1) dock-lift at a time. The Contractor shall zone off the dock-lift that is under repair through the duration of the repair. 2.B. Contractor will coordinate with the Pipe Shop Supervisor prior to work beginning. 2.B.1. The contractor shall provide and install one (1) Fluid Level Gauge. 2.B.2. The contractor shall provide the proper shoring to elevate each Dock Lift while working is being done. 2.B.3. The contractor shall ensure Lock Out Tag Out procedures are followed for each panel that controls each Docklift. 2.B.4. Contractor shall thoroughly clean the direct and indirect areas where work was performed. 2.B.5. Upon completion contractor will contact Sergio J. Cacho, Contract Officer s Representative to inform of the completion of service. 2.C. Contractors must have and provide all necessary tools, equipment, and Personal Protective Equipment (PPE) to perform the work safely, effectively, and timely. Tools, equipment, and PPE must comply with the requirements of OSHA Standard 29 CFR 1910. 2.C.1. Contractor (general or sub-contractor) is required to have completed and provide current safety training certifications: 2.C.1.a. Superintendent: 30-hours Construction Safety course. 2.C.1.b. Tradesmen: Occupational Safety and Health Administration (OSHA) 10-hour Construction Safety training 2.D. Contractor shall obtain all necessary licenses required to perform this work. Contractor shall take all precautions necessary to protect persons and property from injury or damage during the performance of this contract. Contractor shall be responsible for any injury to himself/herself, his/her employees, or others, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by him/her or his/her employees fault or negligence. 2.E. Changes. The awarded Contractor is advised that only the Contracting Officer, acting within the scope of the contract has the authority to make changes which affect the contract in terms of quality, quantity, price or delivery. In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment shall be made in the contract price to cover any increase in costs incurred as a result thereof. 2.F. Parking. It is the responsibility of Contractor personnel to park only in designated parking areas. Parking information is available from the VA Police. The VA shall not validate or make reimbursement for parking violations of the Contractor s personnel under any circumstances. 2.G. Identification Badges: All Contractor personnel are required to wear identification (I.D.) badges during the entire time they are on the VA Healthcare System grounds. I.D. badges must have an identification picture and shall state the name of the individual and the company represented. 2.H. Smoking is strictly prohibited on the grounds of any VHA facility. Per VHA Directive 1085 dated March 5, 2019, it is VHA policy that all VHA health care facilities, including hospitals, community clinics, administrative offices and Vet Centers, will be smoke-free for patients, visitors, contractors, volunteers, and vendors effective October 1, 2019. There will no longer be designated smoking areas. 2.I. Insurance Coverage. 2.I.1. The Contractor agrees to procure and maintain, while the contract is in effect, Workers Compensation and Employee s Public Liability Insurance in accordance with Federal and State of California and/or Nevada laws. The Contractor shall be responsible for all damage to property, which may be done by him, or any employee engaged in the performance of this contract. 2.I.2. The Government shall be held harmless against any or all loss, cost, damage, claim expense or liability whatsoever, because of accident or injury to persons or property of others occurring in the performance of this contract. 2.I.3. Before commencing work under this contract, the Contracting Officer shall require the Contractor to furnish certification from his/her insurance company indicating that the coverage specified by FAR 52.228-5 and per FAR Subpart 28.307-2 has been obtained and that it may not be changed or canceled without guaranteed thirty (30) day notice to the Contracting Officer. 2.I.4. Contractor is required to provide copies of proof of Workers Compensation and Employee Public Liability Insurance within fifteen (15) calendar days after notification of contract award. 2.J.1. All work shall be scheduled during normal work hours (6:00 AM to 6:00 PM) Monday Friday, Pacific Standard Time (PST), excluding Federal Holidays. Work shall not impact patient care services. 2.J.2. Weekend (Saturday or Sunday) or after hours (6PM-6AM, Pacific Standard Time (PST)) work must be approved by the COR assigned to the Task Order. 2.J.3. Any overtime or holiday pay that may be entitled to the Contractor s employees shall be the sole responsibility of the Contractor and shall not be billed to nor reimbursed by the Government. 2.J.4. If a holiday falls on Sunday, the following Monday shall be observed as the legal holiday. When a holiday falls on a Saturday, the preceding Friday is observed as a legal holiday by U.S. Government agencies. Also included, would be any other day specifically declared by the President of the United States of America to be a National Holiday.
