Lebanon VA Medical Center 1700 S. Lincoln Ave. Lebanon PA 17042 2. Berks County VA Community Clinic 2762 Century Blvd. Suite 120, Wyomissing, PA 19610 3. Cumberland County VA Community Clinic 5070 Ritter Road Rossmoyne Business Park Mechanicsburg, PA 17055 4. Lancaster County VA Community Clinic 212 Willow Valley Lakes Drive, Suite 208 Willow Street, PA 17584 5. York County VA Community Clinic 2251 Eastern Blvd. Suite 101 York, PA 17402 The service shall include Maintenance and Repairs to Fitness Equipment as cited in the Statement of Work. The anticipated period of performance is 05/01/2021 to 04/30/2022, with four, one-year option periods. The NAICS code is 811219 and the PSC code is J065, the size standard is 20.5 million.
