- 1. This is a combined synopsis/solicitation for commercial services/items prepared in accordance with the format in FAR Subpart 12.6 in conjunction with FAR Part 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. 1.A. Project Title: Liquid Chillers Preventive Maintenance 1.B. Description:� Quarterly and Annually Preventative Maintenance for liquid chillers located in buildings 126, 138, 160, and 161. 1.C. Project Location:� 5901 East 7th Street, Long Beach, CA, 90822 1.D. Solicitation Type - Number: Request for Quotes (RFQ) - 36C26221Q0537 1.E. Set Aside: This solicitation is 100% set aside for Small Business Entities 1.F. Applicable NAICS code: 238220 Plumbing, Heating and Air Conditioning Contractors. 1.G. Small Business Size Standard: $16.5 Million 1.H. Type of Contract: Firm Fixed Price 1.I. Period of Performance: Base year plus four option years 1.J. Wage Determination: SCA Wage Determination No. WD 2015-5613, revision 16 2. Scope of Work.� VA Long Beach Healthcare System is seeking a contractor to perform scheduled Quarterly and Annually Preventative Maintenance for liquid chillers located in buildings 126, 138, 160, and 161. 2.A. Specific Requirements. 2.A.1.� Contractor shall provide all Labor, Parts, Materials, Equipment, and Supervision required to perform all work. 2.A.2 Contractor is required to have completed the Occupational Safety and Health Administration (OSHA) 10-hour Construction Safety training and 30-hours Construction Safety courses. 2.A.3.� Contractor shall check-in with the Contracting Officer�s Representative (COR) upon arrival at the VHA Long Beach Healthcare System. 2.A.4.� Contractor and Employees are required to wear Temporary ID Badges that are issued from the VHA Police Department while on VHA property. 2.A.5.� Contractor will always be required to wear all Personal Protective Equipment (PPE). 2.A.6.� Collect, contain, and dispose of all debris resulting from the overall scope of the project according to VHA Regulations. 2.A.7. Contractor shall thoroughly clean the direct and indirect areas where work was performed. 2.B.� Contractor will coordinate with Contract Officer�s Representative (COR) Mauricio Ruiz and/or HVAC Shop personnel prior to work beginning. 2.B.1.� Contractor shall thoroughly clean the direct and indirect areas where work was performed. 2.B.2.� Upon completion contractor will contact Mauricio Ruiz, Contract Officer�s Representative to inform of the completion of service. 2.C. Scope of Work: This service will include the following Quarterly and annually schedule according to the manufacture specifications in Schedule B tasking in the attached Statement of Work. 2.D. Contractor shall obtain all necessary licenses required to perform this work.� Contractor shall take all precautions necessary to protect persons and property from injury or damage during the performance of this contract.� Contractor shall be responsible for any injury to himself/herself, his/her employees, or others, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by him/her or his/her employees� fault or negligence. 2.E. Changes.� The awarded Contractor is advised that only the Contracting Officer, acting within the scope of the contract has the authority to make changes which affect the contract in terms of quality, quantity, price or delivery.� In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment shall be made in the contract price to cover any increase in costs incurred as a result thereof. 2.F. Parking.� It is the responsibility of Contractor personnel to park only in designated parking areas.� Parking information is available from the VA Police. The VA shall not validate or make reimbursement for parking violations of the Contractor�s personnel under any circumstances. 2.G. Identification Badges:� All Contractor personnel are required to wear identification (I.D.) badges during the entire time they are on the VA Healthcare System grounds. I.D. badges must have an identification picture and shall state the name of the individual and the company represented. 2.H. Smoking is strictly prohibited on the grounds of any VHA facility. Per VHA Directive 1085 dated March 5, 2019, it is VHA policy that all VHA health care facilities, including hospitals, community clinics, administrative offices and Vet Centers, will be smoke-free for patients, visitors, contractors, volunteers, and vendors effective October 1, 2019. There will no longer be designated smoking areas. 2.I. Insurance Coverage. 2.I.1. The Contractor agrees to procure and maintain, while the contract is in effect, Workers Compensation and Employee�s Public Liability Insurance in accordance with Federal and State of California and/or Nevada laws. The Contractor shall be responsible for all damage to property, which may be done by him, or any employee engaged in the performance of this contract. 2.I.2. The Government shall be held harmless against any or all loss, cost, damage, claim expense or liability whatsoever, because of accident or injury to persons or property of others occurring in the performance of this contract. 2.I.3. Before commencing work under this contract, the Contracting Officer shall require the Contractor to furnish certification from his/her insurance company indicating that the coverage specified by FAR 52.228-5 and per FAR Subpart 28.307-2 has been obtained and that it may not be changed or canceled without guaranteed thirty (30) day notice to the Contracting Officer. 2.I.4. Contractor is required to provide copies of proof of Workers Compensation and Employee Public Liability Insurance within fifteen (15) calendar days after notification of contract award. 2.J.1.� All work shall be scheduled during normal work hours (6:00 AM to 6:00 PM) Monday � Friday, Pacific Standard Time (PST), excluding Federal Holidays. Work shall not impact patient care services.� 2.J.2.� Weekend (Saturday or Sunday) or after hours (6PM-6AM, Pacific Standard Time (PST)) work must be approved by the COR assigned to the Task Order. 2.J.3.� Any overtime or holiday pay that may be entitled to the Contractor�s employees shall be the sole responsibility of the Contractor and shall not be billed to nor reimbursed by the Government. 2.J.4.� If a holiday falls on Sunday, the following Monday shall be observed as the legal holiday. When a holiday falls on a Saturday, the preceding Friday is observed as a legal holiday by U.S. Government agencies.
