- Page 6 of 6 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 -- Streamlined Procedures for Evaluation and Solicitation for Commercial Items, in conjunction with FAR -- Part 13 Simplified Acquisition Procedures; and supplemented with additional information included in this notice. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2021-05, dated March 10, 2021. Solicitation 36C26221Q0603 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award contract. This procurement will be issued under Full and Open Competition. NAICS Code is 811219 and the Small Business Administration s (SBA) size standard is $22.0 Million. Due to time constraints, responses to this RFQ shall be due on Tuesday, April 13th, 1300 PST. Quotes must be submitted electronically via email to Daniel Ruiz, Contracting Officer at Daniel.Ruiz@va.gov. Offeror s are advised to review and monitor the Beta.Sam website for the most up to date information, including all amendments. The Government is seeking a contract to acquire Excelsior Tissue Processor Maintenance & Services. This solicitation is seeking to fulfill the following statement of work: Statement of Work Background. The obligation is a request to establish an annual service agreement to provide preventive maintenance (PM) support for the following tissue processors: Product Serial # Service Type TS Excelsior ET58291803 Preventive Maintenance SERVICE/WORK TO BE PERFORMED: The contractor shall perform on-site preventive maintenance (PM) inspection on equipment as provided for in the contract. Preventive maintenance will be accomplished in accordance with the Original Equipment Manufacturer (OEM) recommendations. Contractor must use OEM certified parts and service engineer/technicians must be OEM trained and certified on all equipment included in the contract. Contractor must provide toll free phone support 24 hours a day, seven days a week. Service calls with on-site response within next business day (Monday to Friday 8:00 AM to 4:30 PM excluding holidays). Response: The contractor shall consider ALL requests for repairs/services as emergencies unless a statement to the contrary is made by the Point of Contact (POC) or designee. A certified service representative adequately equipped with tools, test instruments, service manuals and repair parts will arrive at the NMVAHCS within the designated response time. The service representative may contact VA staff by phone before arrival at the NMVAHCS. PERFORMANCE: Estimated Period of Performance: 4/13/2021 to 4/12/2026, with four (4) one-year option periods. This will be a 5 year terms contract. Hours: Normal duty hours are 8:00 a.m. through 4:30 p.m., Monday through Friday, not including federal holidays. Preventive maintenance will be scheduled with a contact person as assigned by the Point of Contact (POC) or designee. This PM work must be completed in the month specified during the request unless another date is agreed upon by designee. For any repairs or service that will be performed during the normal working hours of 8:00 a.m. through 4:30 p.m. the Vendor shall check-in with the POC or designee before proceeding to the job site. Noncontract Charges: The contractor shall not perform any service that will result in additional charges without prior approval from the Contracting Officer. Payment: Payment will be made upon receipt of a properly prepared invoice which references the period for which payment is due, the contract number and/or assigned purchase order number. The invoice must include the correct payment address. Waste Removal: The contractor is responsible to remove and properly dispose of all waste related to the services provide on-site at NMVAHCS. Failure to carry out the above procedures will be considered as a failure to document the service visit and will constitute a failure to perform. All shipping for parts will be included in the cost of the contract. DOCUMENTATION: Service reports will be provided to NMVAHCS for all service activities including preventive maintenance, emergency repair, and safety inspections, and must contain the following information: Date(s) and time period of service. Complete description of equipment serviced including model number and serial number. Complete description of equipment services performed including upgrades. Complete list of all materials, parts, or assemblies installed or replaced including serial numbers of parts where applicable. A statement of the complaint that initiated the service call. REMOVAL OF EQUIPMENT: Whenever possible, all repairs and preventive maintenance will be performed at the NMVAHCS. When that is not possible, the POC or designee may give approval for service work to be done at another location. All transportation charges will be borne by the contractor. VA property may not be removed from the NMVAHCS prior to execution, and approval of all documents as required by Logistics Service. Requirements for service reports for designated equipment not at this location are the same as for work done on-site. Service reports for incoming equipment will be presented for signature at the time of delivery. If equipment is returned via commercial carrier, the service report may be included with the shipment. GOVERNMENT REPAIRS: Government employees may work on contract equipment only with authorization of the contractor or following the failure of the contractor to respond. The government is not obligated to perform any work on contract equipment. GUARANTEE: The contractor will guarantee workmanship and all parts furnished by the contractor for a period of not less than 90 days. EQUIPMENT: EQUIPMENT MODEL: Thermo Scientific Excelsior AS SN: ET58291803 Place of Performance. VA Albuquerque Healthcare System 2100 Ridgecrest Dr, SE Albuquerque, NM 87108 Period of Performance. One (1) Year Base Period starting on 4/13/2021 with the Government having the option to exercise (4) four (1) one-year renewal periods in conjunction with 52.217-9. Base Year 4/13/2021 to 4/12/2022 Option Year 1 4/13/2022 to 4/12/2023 Option Year 2 4/13/2023 to 4/12/2024 Option Year 3 4/13/2024 to 4/12/2025 Option Year 4 4/13/2025 to 4/12/2026 The information identified above is intended to be descriptive, not restrictive and to indicate the quality that will be satisfactory. CONTRACT CLAUSES AND PROVISIONS 52.212-2 Evaluation Commercial Items (Oct 2014) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Technical acceptability shall be established by review of each submitted quote, attachments, and/or information by the designated technical evaluator/s and verification that any brand name or equal to items"" meet all the salient characteristics included in this solicitation. No other rating or quantitative value other than acceptable or unacceptable will be assigned. Once technical acceptability has been established, the Contracting Officer shall award to the lowest price. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. (End of clause) 52.217-6 - Option for Increased Quantity (Mar 1989) The Government may increase the quantity of supplies called for in the Schedule at the unit price specified. The Contracting Officer may exercise the option by written notice to the Contractor within 10 Days not to exceed a 50% of any and all line items within Attachment A. Delivery of the added items shall continue at the same rate as the like items called for under the contract, unless the parties otherwise agree. (End of clause) 52.217-8 - Option to Extend Services (Nov 1999) The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 60 days of contract expiration. (End of clause) 52.217-9 - Option to Extend the Term of the Contract. (Mar 2000) (a) The Government may extend the term of this contract by written notice to the Contractor within 30 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension. (b)If the Government exercises this option, the extended contract shall be considered to include this option clause. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 60 Months.
