- Page 3 of 3 Good Morning, The Department of Veterans Affairs, VISN 6 Network Contracting Office (NCO) is conducting Market Research through this sources sought notice to help determine the availability of qualified Businesses, registered under NAICS code 334118, capable of providing ScriptPro Electronic Signature Device including install/support for Durham VAMC: Electronic Signature Device INCLUDES INSTALL/SUPPORT: Quantity 4 All respondents shall indicate the following: Whether or not they are small business manufacturers of the products they wish to offer, or Whether or not they are distributors of a small business manufacturer. Indicate if product is Buy American Act or Trade Agreement Act compliant. Identify any other companies owned (wholly or in-part) by the owners of this business who provide goods or services under the same or a related NAICS code. Identify any other companies owned (wholly or in-part) by the owners of this business who provide goods or services that are registered under the same or a related NAICS code with the Center for Veterans Enterprise (CVE); Provide the certification type (SDVOSB/VOSB), Federal Identification Number, and state of incorporation for each. Do you plan on responding to a solicitation for this requirement with a Joint Venture utilizing multiple owned companies as majority or non-majority owner? Yes ___No ___ If yes, please identify which companies are considering a Joint Venture and the ownership of each company. Please provide technical specs/Brochure of brand name or equal to equipment. Failure to provide the above information may result in the potential offeror being considered incapable of meeting the VA s needs If the contractor can provide the above items, the contractor shall provide a letter from the manufacturer stating they are an authorized distributor/licenser of their products. Failure to submit an authorized distributor letter shall result in the product(s) potentially offered to be considered grey market and shall not be considered technically acceptable. If the contractor can provide the above product and services, please indicate in your response that the above requirements can be met and provide a brochure of its product. The capability statements received from this market research are for planning purposes only and will assist the Government in planning its acquisition strategy. This is strictly market research and the Government will not entertain questions concerning this market research. The Government will not assume any costs incurred by the contractor in the preparation of responses. STATEMENT OF WORK (SOW) A. GENERAL GUIDANCE 1. Installation of ScriptPro Electronic Signature Pads at the Durham VA Medical Center. This work will cover all labor materials, and equipment required to install ScriptPro equipment and ensure that it is properly running. All electrical, wiring, and mounts must be in compliance with general VA requirements, 2. Scope of Work: Installation, setup, and training on all purchased ScriptPro equipment. The work covered under this contract consists of furnishing all labor, materials, tools, equipment, and supervision necessary to provide for the successful installation of the equipment at the Durham VAHCS. Work should be completed with minimal impact on staff and patients, when possible. Costs should also include all required work to provide for a final finish. 3. Background: Pharmacy is submitting a request for purchase and installation of the required ScriptPro technology at Durham VAHCS. 4. Performance Period: Completion of project as early as possible after purchase of equipment. 5. Type of Contract: A firm-fixed-price. B. GENERAL REQUIREMENTS This project will provide all necessary equipment, connective wiring, and brackets as required for the installation electronic signature pads at the Durham VAHCS. The project consists of: Providing all supervision, labor, materials and equipment required to complete project. Contractor shall consult with Durham VAHCS engineering, Biomed, and IT when connecting electrical and network. Provide corrective maintenance on any issues encountered immediately following installation. Durham VAHCS will direct where the required equipment will be placed. Contractor shall be responsible for providing repairs of faulty installations within 72 hours (at no additional cost to the Durham VAHCS). C. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES: Special Requirements All electrical and networking data will be inspected by Durham VAHCS engineering to ensure contract compliance and safety. A site visit will be scheduled by the contracting officer. Contractor attendance at the site visit is optional. Engineering Requirements Work must be compliant with all appropriate electrical, building, safety, OSHA, and other codes and regulations applicable to the Durham VAHCS. D. GOVERNMENT RESPONSIBILITIES: The Durham VAHCS will be responsible for supplying power source data network to connect and power contracted wiring. E. CONTRACTOR EXPERIENCE REQUIREMENTS: QUALIFICATIONS/CERTIFICATIONS The contractor shall be qualified and have the required levels of professional and technical experience to perform the work required by this statement of work. F. EQUIPMENT: All equipment, tools, etc., shall be properly maintained while on the project and shall be adequate, in quality and capacity, for proper execution of the work. G. SCHEDULING AND HOURS OF WORK All work in areas involved shall be scheduled with the VA Engineering Service representative. Work shall be scheduled for proper execution to completion of this contract. H. SITE VISITS Job site shall be visited to verify all existing conditions and dimensions and the extent of work involved with the VA Engineering Service Representative. Visits to the site may be made by appointment only, during regular business hours, with the VA Contracting Officer Representative (COR). I. INSPECTIONS: All work shall be inspected during installation and at completion by the VA technical representative. J. SAFETY REGULATIONS: All applicable safety regulations shall be observed during all work of this contract. The VA Safety Officer shall have full authority to see that the contractor obeys all safety rules and regulations relative to the fulfillment of this contract. Durham VAHCS will be responsible for providing the necessary barricades/signage where required.
