- REQUEST FOR INFORMATION VETERANS INTEGRATED SERVICE NETWORK (VISN) 21, VA PACIFIC ISLANDS HEALTH CARE SYSTEM (VAPIHCS) Description 1.1 The Network Contracting Office 21 in support of the Veteran Health Care Administration is seeking information on available sources for the pickup, delivery, storage and routine service/refurbishment of Durable Medical Equipment (DME) to veteran patients served by VA Pacific Islands Health Care System (VAPIHCS) Prosthetic Service. 1.2 THIS IS A REQUEST FOR INFORMATION (RFI) ONLY.� This RFI is issued solely for information and planning purposes � it does not constitute a Request for Proposal (RFP) or a promise to issue an RFP in the future.� This request for information does not commit the Government to contract for any supply or service whatsoever.� Further, the VHA is not at this time seeking proposals and will not accept unsolicited proposals.� Respondees are advised that the U.S. Government will not pay for any information or administrative costs incurred in response to this RFI; all costs associated with responding to this RFI will be solely at the interested party�s expense.� Not responding to this RFI does not preclude participation in any future RFP, if any is issued.� If a solicitation is released, it will be synopsized on the Contract Opportunities website https://beta.sam.gov/ .� It is the responsibility of the potential offerors to monitor these sites for additional information pertaining to this requirement. 2.0 Background The Government intends to award a minimum of one (1) firm fixed-priced Indefinite Delivery, Indefinite Quantity contract (IDIQ) contract for each/all Healthcare System facilities as identified below. The period to be covered by this contract will contain one base period (TBD 2021), with four (4) one- year options to extend. The contractor will provide services for VA Pacific Islands Health Care System patients at sites listed below VA Pacific Islands Health Care System Facilities Guam VA Clinic 498 Chalan Palasyo Agana Heights, GU 96910-6427 Faleomavaega Eni Fa�aua�a Hunkin VA Clinic Fiatele Teo Army Reserve Building Pago Pago, AS 96799 Saipan VA Clinic Marina Heights Business Park � Garapan Medical Associates of the Pacific MH-II Building Suite 100 and 206 Saipan MP 96950-9998 3.0� Requested Information If interested, please provide the following information.� Failure to provide the following information completely and accurately may result in a determination that your company is not a viable source: Name of Company Point of Contact Name, phone number, and email address DUNS number Socio-economic status such as Service-Disabled Veteran Owned Small Business, VOSB, Women-Owned Small Business, etc. should be included. SDVOSB/VOSB status will be verified using VetBiz. Capability statements that include the following: Identify geographical areas your company currently services? Indicate prior experience for same/similar service in these geographical areas.� If the experience is through government contract, please identify the contract number. Identify generally what administrative resources your organization has in the areas identified.� For example, identify offices located in the identified region. 4.0 Responses Interested parties are requested to respond to this RFI via email with a capabilities statement and provide their Business type (large business, small business, small disadvantaged business, 8(a)-certified small disadvantaged business, HUBZone small business, woman-owned small business, very small business, veteran-owned small business, service-disabled veteran-owned small business). 5.0 Questions Questions regarding this announcement shall be submitted in writing by e-mail to the Contracting Officer, Victoria Torres at victoria.torres@va.gov.� Verbal questions will NOT be accepted.� Questions will be answered by posting answers to the Contract Opportunities Beta Sam website. 6.0 Summary THIS IS A REQUEST FOR INFORMATION (RFI) ONLY to identify sources that can provide the pickup, delivery, storage and routine service/refurbishment of Durable Medical Equipment (DME). The information provided in the RFI is subject to change and is not binding on the Government.� The VHA has not made a commitment to procure any of the items discussed, and release of this RFI should not be construed as such a commitment or as authorization to incur cost for which reimbursement would be required or sought.
