- The purpose of Amendment 36C24421Q0244 0001 is detailed as follows: A - The response time is extended from 10:00 am 4-15-2021 to 10:00 am 4-23-2021. B Schedule a site visit A site visit will be conducted on Monday, April 19 at 7:00am. Vendors shall meet at Building 15 at the Corporal Michael J Crescenz Veterans Affair Medical Center, Philadelphia, PA. Site visit poc is William Lent, 856-777-0729 C Include VAAR 852.219-74 852.219-74 Limitations on Subcontracting Monitoring and Compliance. As prescribed in 819.7203(a) insert the following clause: LIMITATIONS ON SUBCONTRACTING MONITORING AND COMPLIANCE (JUL 2018) (DEVIATION) (a) This solicitation includes _______52.219-6_________________________ [Fill-in clause/provision] [Note: Contracting Officers must fill-in the applicable clause required to be included in sole-source and/or set-aside acquisitions (e.g., VA Acquisition Regulation (VAAR) 852.219-10, VA Notice of Total Service-Disabled Veteran-Owned Small Business Set-Aside, 852.219-11, VA Notice of Total Veteran-Owned Small Business Set-Aside; FAR 52.219-6, Notice of Total Small Business Set-Aside).] (b) Accordingly, any contract resulting from this solicitation is subject to the limitation on subcontracting requirements in 13 CFR 125.6. The Contractor is advised that in performing contract administration functions, the Contracting Officer may use the services of a support contractor(s) retained by VA to assist in assessing the Contractor s compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to Contractor s offices where the Contractor s business records or other proprietary data are retained and to review such business records regarding the Contractor s compliance with this requirement. (c) All support contractors conducting this review on behalf of VA will be required to sign an Information Protection and Non-Disclosure and Disclosure of Conflicts of Interest Agreement to ensure the Contractor s business records or other proprietary data reviewed or obtained in the course of assisting the Contracting Officer in assessing the Contractor for compliance are protected to ensure information or data is not improperly disclosed or other impropriety occurs. (d) Furthermore, if VA determines any services the support contractor(s) will perform in assessing compliance are advisory and assistance services as defined in FAR 2.101, Definitions, the support contractor(s) must also enter into an agreement with the Contractor to protect proprietary information as required by FAR 9.505-4, Obtaining access to proprietary information, paragraph (b). The Contractor is required to cooperate fully and make available any records as may be required to enable the Contracting Officer to assess the Contractor s compliance with the limitations on subcontracting or percentage of work performance requirement. (End of clause) D - Provide responses to the following questions received: Can the companies team with Gamewell/FCI Dealer to perform programming services on the contract? We would have a Gamewell Certified Tech perform programming/emergency repair service. Limitations on Subcontracting applies per clauses: 52.219-14, 852.219-74 and 52.219-6 Can you confirm the number of current fire alarm systems trouble & supervisory signals that are showing on the systems? Will the correction of the current fire alarm troubles and supervisory faults be done by using X002 and X003. Repairs are ongoing as new troubles come in randomly. SOW Section 2.1 Is the contractor to cover the cost (up to $2,500) of equipment that fails during inspection? If so when would line items X002 and X003 be used for this contract? The contractor is responsible for providing all materials, components, labor, tools, test instruments, and travel necessary to complete work under this contract for the fire protection and monitoring systems as covered in CLINS 0001, 1001, 2001, 3001 and 4001 in which the contractor takes total service responsibility for the equipment identified in the contract and upon acceptance of this contract accepts the equipment, as-is. As such, the monthly firm fixed price includes all service, testing and inspections requirements, maintenance, repairs, parts replacements (up to $2,500) and routine or emergency service calls including premium time. CLIN 0002, 1002, 2002, 3002, 4002 and CLIN 0003, 1003, 2003, 3003, and 4003 will be used on a case by case basis. Will reports need to be completed in Joint Commission Format? Yes. Ensure Section 9.2.1.1.1 is followed Will contractor need to provide reporting software or will VA provide reporting software? Contractor provide reporting software in Joint Commission format Ensure Section 9.2.1.1.1 is followed Section 8.13 Is the contractor to install a new Graphical User Interface (GUI)? Yes Where is the GUI to be installed? See Section: 8.13.4.3 all sections Is all current equipment compatible with GUI or is there other equipment that will need to be provided? Contractor provide and install GUI to tie into existing addressable system. See: 8.13 all sections related to GUI. If the VA can t confirm will a site visit be allowed to verify site conditions? Site Visit authorized Is there going to be a separate line item for GUI install? Yes Is there a contract for the fire sprinkler inspections? If so are we required to be present during the fire sprinkler inspections of tamper and flows? Please reference the solicitation. Inspections are referenced throughout the SOW. How will 6yr and 12yr testing be handled? Are we to provide a quote when these services are required? Base year of the contract will be the 6 year test. The 12 year test will be under a separate contract How many fire pumps are to be tested monthly?? 2 Will the Automatic Fire Door require an NFPA 72 inspection or NFPA 80 Inspection? In accordance with NFPA 72 and 80. Will testing frequency be IAW with the most current NFPA Edition, Joint Commission & VHA Guidance? Yes Section 8.5.1.1.3.6 to 8.5.1.1.3.8 Was the 5 year Internal Inspection and Standpipe Flow Test Completed in June 2020? Yes How many gauges will need to be replaced in June 2022? 150 Will there be testing that needs to be performed after hours? Can you provide a list of inspections that need to be performed after normal hours? FM200 Systems, UL300 Systems, duct detectors for air handlers, hydrants, and elevators Will the VA provide an escort and/or keys/badge for inspection team? VA will provide escort and badging. No keys will be provided. I see at page 26 is a price/cost schedule. I am clear on the unit price and amount for the 5 year portion, my question is about the lines in reference to the repairs (0002 and 0003), I really am not sure how to put numbers in the pricing section as that is not a known cost. The cost of any repairs would not be known until the problem is identified and able to be priced out. You do not need to provide pricing for repair CLINs 0002, 1002, 2002, 3002, 4002, 0003, 1003, 2003, 3003, and 4003. These CLINs will be used on a case by case basis. For the actual submission of the proposal starting at page 45, I understand completing 1 document for each of the sections B,C,D and E. For section D, Would this would be the same as section C just removing any company or employees names? Yes, Section D is the same as Section C. Any identifying information from Section C should be removed for Section D. Moving past that the pages have check boxes and sections that I believe need to be completed by me, which section page document would I attach those too. This should be included with Section I Administrative/Regulatory.
