The North American Industry Classification System or NAICS code is 561320 with a small business size standard of $30.0 M.� The Request for Quotations No. 75H71021Q00018 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be issued on or by 04/15/2021.� Evaluation factors may include: Residency in OB/GYN Contractor�s experience of twenty-four (24) months 3.�������� OB/GYN Board certified or eligible 4.�������� Provides full-scope OB/GYN care (including high-risk OB, laparoscopic GYN surgery) 5.�������� Licenses/Certifications (BLS, NRP required) 6.�������� Interview with Contractor against Statement of Duties, Performance Work Statement, and Contractor Profile 7.�������� Review of references (at least 2) 8. ������� Submittal of Option CLIN 9.�������� Price Quotes will be due on or by 04/26/2021.� This acquisition is unrestricted.
