Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is 36C24721Q0594 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. The associated NAICS code is 492110, Couriers and Express Delivery Services. This is set-aside for Service-Disabled Veteran Owned Small Business. The requirement is operating Community Based Outpatient Clinics (CBOC) in Milledgeville, Georgia and Tifton, Georgia to better serve the Veterans in and around those areas. Please submit all quotes NLT April 23, 2021 10:00 am eastern standard time to sheryl.harris5@va.gov; late submissions will not be accepted. NOTE Evaluation Criteria OFFEROR S QUALIFICATIONS: Award will be made to the lowest price technically acceptable offer along with receipt of all VA clearance forms, HIPPA/OSHA training certificates received with Offerors proposal. Technical acceptability will be based upon offerors submitting enough evidence of adequate HIPPA and OSHA training certificates, equipment, personnel and necessary experience to establish responsibility and integrity to meet the requirements of this contract with offeror s proposal prior to award.  Any offeror, who does not have adequate training, experience, personnel and/or equipment, in the opinion of the Contracting Officer, will be rejected.  To assist in this determination, offeror is required to furnish the following information: Number of years experience performing the type of services indicated in the SOW Number of personnel regularly employed on a full-time basis HIPPA and OSHA training certificates of all personnel submitted with Offeror s proposal Submit all personnel VA clearance forms 0710, OPM form 306, contractor background investigation request form and self-certification of continuous service forms along with proposal Offerors who have not previously provided the same or similar type services at a VA Medical Center or healthcare facility are required to furnish for evaluation purposes, the names, addresses and telephone numbers of three (3) companies for whom exact services have been furnished. Award shall be based on the lowest price technically acceptable basis and receipt of all required forms, certificates the submission of all required documentation at the time of along with all training certificates of personnel being provided by offeror during the solicitation period. Award will be made on the basis of the lowest evaluated price of quotations meeting or exceeding the acceptability standards for all non-cost/price factors. **ALL factors must be rated ACCEPTABLE to be considered for award. Each factor will be scored as Acceptable or Unacceptable as defined below. Each quotation will be evaluated against the SOW and the stated evaluation criteria using the same ""Lowest Price Technically Acceptable evaluation standards. Acceptable ALL of the minimum acceptable criteria are clearly met by the proposal. The offerors proposal meets the technical capability requirements defined in the SOW. NOTE: Once the proposals have been determined to be ""technically acceptable,"" award will based on cost/price along with receipt of all required documents, VA forms and training certificates only. Unacceptable Not all of the minimum acceptable criteria are met by the proposal. An unacceptable proposal contains one or more deficiencies. Proposal fails to meet specified minimum technical capability requirements defined in the SOW. The following table will be used for each proposal received to score each LPTA factor as to whether it is Acceptable or Unacceptable. Technical Factors Offeror Technically Acceptable Offeror shall submit current Business License of legality authorizing them to Sensitive Document Destruction Offeror shall submit proof of Liability Insurance Offeror shall submit a Statement of Capabilities, Knowledge and Skills Relevant Experience/Past Performance: The Relevant Experience/Past Performance evaluation will assess the relative risks associated with offeror s likelihood of success in performing the requirement as indicated by that offeror s record of Relevant Experience/Past Performance. Offeror must provide at least three (3) customers of reference which shall demonstrate relevant past performance. These references shall contain name, address of facility, contact name, phone number, dates of service, and contract number, if applicable. The experience shall be of similar size and scope. Offeror will have 5 years or more experience in VA courier services.
