10 DISCLAIMER This Sources Sought is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this notice is marked as proprietary and will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this notice. This notice is being issued in an attempt to determine the level of interest among all socioeconomic groups. Responses will assist VA planners in determining an appropriate acquisition and set-aside strategy which best fits VA requirements. This action is to perform market research to determine if there is a sufficient number of qualified (1) Service-Disabled Veteran Owned Small Business; (2) Veteran Owned Small Business; (3) Small or emerging small business firms; or (4) large business firms to set aside a multiple award contract. The Contractor shall be responsible for providing all components of the requirement necessary to provide an indefinite delivery/indefinite quantity contract for non-emergent ground ambulance services to provide transportation to patients at the Wilkes-Barre VA Medical Center, twenty-four hours a day, seven days a week. Contractor shall provide all vehicles, personnel, management, supplies, transportation, equipment and reports necessary to cover ground ambulance transport service. Contractor care shall cover the full range of wheelchair, van service, paramedic and ambulance services provided in the local community. Paramedics responding to codes will bring equipment/supplies required by state regulations, and Advanced Life Support (ALS) equipment and medication to include, but not limited to, cardiac monitoring, intubation equipment, automated external defibrillator (AED), video laryngoscope, pulse oximetry, glucometer, portable oxygen, protective clothing and written procedures. The Contractor shall have available the appropriate number of vehicles to provide services in a timely manner as outline by the contract. Performance shall be in accordance with the requirements contained in this SOW and standards of the Pennsylvania DOH, Bureau of Emergency Medical Services, and The Joint Commission. The anticipated period of performance is five (5) years, with task orders to be issued as services are required. All interested firms who can meet the requirement should respond, in writing, and provide the following: 1. Describe your interest, including your capability to provide this service 2. Indicate which socioeconomic business category for which you qualify. 3. CAGE and DUNS codes. 4. Contract numbers of other contracts for references in the past 3 years. The NAICS Code 621910 is applicable to this acquisition, with a Business Size Standard of $16.5M. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Systems for Award Management (SAM) database prior to award of a contract.
