- Department of Veterans Affairs, New York Harbor Healthcare System, Network Contracting Office 2, is issuing this Notice of Intent to Sole Source in order to identify any potential sources that have the resources and capabilities to provide a comprehensive vehicle for full preventive maintenance, repair and software updates for three Medivator Advantage Plus Endoscope Reprocessor ADV-1007 and two Medivator Veriscan Leak Detection Systems. SERVICES REQUIRED: Contractor shall provide annual service for preventive maintenance, repair and software upgrade for three Medivator Advantage Plus Endoscope Reprocessor ADV-1007 at the Stratton Veterans Administration Medical Center, 113 Holland Ave, Albany NY 12208 and two Medivator Veriscan Leak Detection Systems at the James J. Peters VA Medical Center, 130 West Kinsgbridge Road, Bronx, NY 10468 during the term of May 01, 2021 through April 30, 2022 with provision for four (4) Option Years. Contractor shall provide services for Medivator Advantage Plus Endoscope Reprocessor and Medivator Veriscan Leak Detection Systesm. INTERESTED & CAPABLE RESPONSES: NCO 2 is seeking responses from businesses that are interested in this procurement and consider themselves to have the resources, rights, and capabilities necessary to provide these services. Please provide all of the following in your response: Business name, business type, socio-economic status (e.g., Veteran-Owned, Woman-Owned, Disadvantaged Small Business, 8(a), etc.), person of contact (including telephone number & email address), GSA Contract number if applicable, and DUNS number. Please also include the North American Industry Classification System (NAICS) Code for which you propose to provide the service under for verification of your size standard. A Capability Statement which provides detailed, specific information that demonstrates understanding of the tasks required and capability to perform the tasks as stated in the attached Statement of Work. Failure to tailor capability statement to this specific requirement may result in your company being determined non-responsive. Responses must be submitted by 3:00 PM EST, Thursday, April 22, 2021. Responses to the information requested above may be submitted via email to Monique.Paltan@va.gov. This Special Notice of Intent to Sole Source is for planning purposes ONLY. The results of this notice may assist in the development of (1) the requirement, and (2) the acquisition strategy (e.g., small business set-aside, full and open competition, etc.). VA assumes no responsibility for any costs incurred associated with the preparation of responses submitted for this notice. STATEMENT OF WORK A. BRACKGROUND: The Department of Veterans Affairs, Stratton VA Medical Center in Albany, NY has three (3) Medivators Advantage Plus Endoscope Reprocessor ADV-1007 units for which is seeking preventive maintenance services. These three units sterilize and properly clean the endoscopes for the Stratton VAMC ensuring the required endoscopes are sterilized and provided to patients in a timely manner. In order to keep these units in optimal functioning order and to reduce downtime due to malfunctioning units, we require full annual service for preventive maintenance, repair and software updates. B. SCOPE OF WORK: The contractor shall provide factory trained technicians and all resources (labor, tools, equipment, travel, parts and services) necessary to accomplish the preventive maintenance, repair and software updates of the three Medivators Advantage Plus Endoscope Reprocessor ADV-1007 units located at the Stratton VA Medical Center. The contractor shall provide remedial service including all labor, travel and parts if required to repair any malfunction of all three units between preventive maintenance calls. Scheduled preventive maintenance will include, but not be limited to, electrical safety testing, lubrication, adjustment, calibration, testing, and replacement of parts which would normally be replaced during the course of a standard PM (as recommended/required by manufacturer procedures), or parts found in the manufacturers PM Kit . The PM procedures will be performed at specifications required by the equipment manufacturer. These procedures shall be presented in writing to the COR. At the conclusion of the preventive maintenance visit, the instrument(s) and/or equipment shall be returned to the operating condition stipulated by the manufacturer's factory specifications. Scheduled preventive maintenance service will be provided annually by the manufacturer in accordance with the contract. Unless a particular time is specified in the contract, the Contractor shall notify the COR within thirty (30) days of acceptance of the contract of the times during which the preventive maintenance inspections are to be performed. The contractor is responsible for scheduling all PM Service visits prior to the end of the month in which they are due. Any deviations from this schedule must be approved in advance by the COR. Contractor shall repair, modify, or correct as necessary equipment deficiencies related to any and all hazard alerts from the manufacturer, FDA, or Veterans Administration, etc. at no additional cost. The contractor shall provide web-based technical support twenty-four (24) hours a day seven (7) days a week as well as live toll-free technical support line during normal operating hours. The contractor shall provide one (1) operator training program for the facility provided by Medivator Clinical Support Specialist. Contractor shall provide access to all updated user manuals and references guides as released by the OEM. The contractor shall respond to the Biomedical Engineering Designee and/or COR with a phone call within two (2) hours, 24 hours a day after receipt of telephoned service call notification. C. CONTRACTOR QUALITY ASSURANCE. The contractor must adhere to guidance and instructions provided in the OEM repair and maintenance manuals. The contractor shall provide documentation of services provided in an Equipment Service Report (ESR). All ESRs shall be submitted to the equipment user for an ""acceptance signature"" and to the COR or Biomedical Engineering designee for an ""authorization signature"". If ESRs are available electronically, a signed authorized copy of the ESR will be sent to COR and Biomedical Engineering designee via email to the addresses. The documentation will include legible detailed descriptions of the services and parts delivered. All records and logs shall be turned over to the COR or Biomedical Engineering Designee at the completion of the work performed. The contractor shall provide a checklist with worksheet originals indicating work to be performed and actual values obtained. Contractor must maintain all operational and technical documentation such as operational and service manuals, schematics, diagnostic software and parts list. Contractor must maintain an Inspection Record Log. For each service visit, a detailed Equipment Service Report will be required. Each service report shall include: Name of the contractor and contracting number. Name of the FSE performing services. Contractor Service ESR number/log number. Date, time (starting and ending), equipment downtime and hours on-site for service call. Identification of equipment to be serviced, ID number, manufacturer s name, device name, model number, serial number and any other manufacturer s identification numbers. Description of problem reported by Biomedical Engineering Section. Signature from the FSE performing services described. Signature from an authorized VA employee who witnessed services described The contractor shall provide written proof that all gauging and measuring equipment used to check and calibrate the parts has been tested in accordance with the Original Equipment Manufacturers (OEM) standards. D. ADDITIONAL REQUIREMENTS: Contractor shall draw attention to, and specifically describe any additional benefits or options that are incorporated in their standard maintenance agreement that are not included in the above specifications. Examples may include remote diagnostic monitoring, discounted labor rates for repairs outside of normal working hours, extended normal working hours, discounted/no charge hardware and/or software upgrades, discounted additional hardware and/or software options, discounted labor rates for emergency repairs, downtime protection guarantees, etc. Brief explanations illustrating why these options are beneficial or how they increase efficiency or productivity, or decrease downtime, should be included. The contractor shall be responsible for informing the COR of any uncorrected deficiencies and noting these on the service ticket. Notation will include the type of deficiency, dated and initialed. Any deficiency which poses a hazard to patients, staff, or other equipment will immediately be called to the attention of the users and the COR. Contractor's service representative shall contact Biomedical Engineering (518) 626-6270 prior to any service performed. Service tickets will be signed and filed in Biomedical Engineering (Bldg. 2, room C52c and GB-02). In lieu of presenting service tickets while on-site, service tickets may be emailed to clark.wetzel@va.gov and luis-enrique@va.gov. Failure to provide signed service tickets will result in non-payment. Work not covered by this contract shall not be performed without first obtaining permission of the COR and/or the Contracting Officer. Bills resulting from additional work performed without prior approval will not be honored. E. CONTRACTOR IDENTIFICATION: The contractor personnel must have in their possession for inspection by VA Police Department a valid state or government issued identification such as drivers license or passport. Personnel may be required to surrender their ID to the VA Police to obtain a temporary day pass at the discretion of VA Police. ID will be returned upon leaving US Government property. Day pass must be worn at all times above the waist, in front, with the face of the card visible. Once issued a temporary identification badge, the contractor s Field Service Engineers are to report to the Biomedical Engineering Department. The contractor will then be escorted by Biomedical Engineering personnel to the respective work site. F. WORKING BUSINESS HOURS: All service and deliveries will be performed during normal business hours of coverage unless proper alternate arrangements have been coordinated beforehand with the COR. Working hours will be Monday to Friday from 8:00 AM EST to 5:00 PM EST excluding Federal Holidays. Federal Holidays which are observed by the federal government are annotated below. G. PERIOD OF PERFORMANCE: The Base Period of Performance will be one (1) year from the date of award of contract with the provision of four (4) Option Years.
