- This is a request for Information: SUBJECT: Request for information (RFI), synopsis and a non-personnel service requirement to provide Surveys for VA Community Living Centers (CLC) and State Veterans Homes (SVH). The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government. This RFI allows potential contractor to submit a non-binding statement of interest and documentation demonstrating their capabilities, and suggestions that will allow the Government to refine its potential course of action. The RFI effort is intended to assess industry service capabilities and solicit feedback for acquisition planning purposes. The strategic Acquisition center (SAC) is requesting response from qualified businesses concerns. The NAICS code for this effort is 541611 PSC is R499. BACKGROUND: There are 135VA CLCs and 5 SCI/D Long Term Centers across the United States and Puerto Rico. (Attachment D) Additional CLCs and/or SCI/D Long Term Centers may be added during the lifetime of this contract; any new CLCs and/or SCI/D Long Term Centers that is opened during the performance period will also need to be surveyed and will be added via bi-lateral modification of the task order. The CLCs offer an array of services including, but not limited to: short stay services such as hospice, respite, skilled nursing and rehabilitation. long stay services such as dementia care. care for veterans with spinal cord injury and disorders; and mental health recovery care for eligible veterans. There are currently 158 State-Owned and Operated Veterans Homes across the United States; this includes all 50 states, territories, tribal lands and possessions of the United States, and the Commonwealth of Puerto Rico. (Attachment E) Additional SVHs may be added during the lifetime of this contract. The State-Owned and Operated Veterans Homes provide only three levels of care to include nursing home care, domiciliary care, and adult day health care. The SVHs offer an array of services including, but not limited to: short stay services such as hospice and palliative care, skilled nursing and rehabilitation. long stay services such as dementia care. Wound care. Mental health services. SPECIFIC MANDATORY TASK AND ASSOCIATED DELIVERABLES. Please see attached Performance Work Statement Section B INSTRUCTIONS: Submit a brief description, five (5) pages or less (including cover letter) that demonstrates how your company can provide an independent, peer review system of accreditation that is widely recognized by federal agencies, state governments, major insurers, and leading professional rehabilitation groups, as well as by consumer and advocacy organizations. Provide your experience in performing these services for other government (federal or states) agency s. Responses should include business size, business type, socio-economic status, and capability.
